Instructions: Go to the Chemical Safety Board website (csb.gov), and view the video: Anatomy of a Disaster
. Answer the following questions:

List three things that could have been done to prevent this accident from occurring
Replace faulty sensors and equipment. So it can make the proper readings and trigger alarms when malfunctions happen.
Prioritize safety over profitable margins/production. Making sure everyone has proper training and proper PPE and plant operations is safe before heading out. Communications between staff. Have meetings before , mid shift, and every shift before leaving. Update next shift coming before exiting the facility. Regardless if someones late or off due to being tired.
2.
Describe the dangers of having non-essential personnel working so close to refinery equipment
Don't have proper training to operate and identify hazards of being around the unit.
Not wearing up to code proper PPE to protect themselves from hazardous material. Which can lead to many life risk/threatening issues. Not being involved in the daily start of meetings. To provide the risk and dangers of working in the unit. Having non-essential personnel working in close proximity to refinery equipment can pose significant dangers due to the inherent risks associated with the refining industry. Refineries deal with highly flammable and hazardous substances, and strict safety protocols are essential to prevent accidents and protect personnel. To mitigate these dangers, refineries must strictly adhere to safety regulations, provide comprehensive training for all personnel, establish clear access controls, and enforce safety protocols to ensure that only essential personnel with the proper training and PPE are allowed in close proximity to refinery equipment.
