Formula for Calculating Peds Maintenance Fluids The following formula must be used: 1 st 1-10 kg x 100 mL Weight of child is >10kg 2 nd 12-20 kg x 50 mL Weight of child > 20kg 3 rd >20 kg x 20 mL Example 1 (Full Maintenance) : A 25 kg child is to receive maintenance fluids over 24 hrs. Determine total volume to be infused and the rate of the infusion (pump rate). 10 kg x 100 mL = 1000 mL 10 kg x 50 mL = 500 mL 5 kg x 20 mL = 100 ml _______ 1600 mL (total volume answer ) ÷ 24 hrs = 66.66 = 67 ml/hr (infusion rate answer) Example 2 (½ Maintenance): The order is to give ½ maintenance fluids to your child who weighs 20 kg to be infused over 24 hours. Determine the total volume to be infused and rate of infusion (mL/hr). Same formula is used 10 kg x 100 mL = 1000 mL 10 kg x 50 mL = 500 mL ______ 1500 mL (total volume) x 0.5(1/2 fluids) = 750 mL (total volume answer ½) 750 mL (total volume) ÷ 24 (infusion time) = 31.25 or 31 mL/hr (infusion rate answer) Example 3 (1.5 Maintenance): The child’s weight is 35 lbs., and the order is to give 1 ½ maintenance fluids over 12 hours. Determine the total volume to be infused and rate of infusion (mL/hr). Same formula is used but first need to convert the 35lbs to kg. 1kg/2.2 lbs. and 35 lbs./1 = 35/2.2= 15.9 kg(wt.) 10kg x 100mL = 1000 mL 5.9 x 50 mL = 295 mL _________ 1295 (total volume) x 1.5 (1.5 fluids) = 1942.5 mL (total volume answer 1.5) 1942.5 ÷ 12 hrs =161.87 or 162 mL/hr (infusion answer) PEDIATR IC FLUID
Practice Questions 1. Your order is to provide 1 ½ maintenance fluids over 24 hours on a child that weight is 22 kg. Determine the total volume and the rate of infusion. 2. Your order is to provide ¾ maintenance fluids over 12 hours for. Your patient with a weight of 30 kg. Determine the total volume and the rate of infusion. 3. Infuse ¼ maintenance fluids over 24 hours for your patient weight of 10 lbs. Determine total volume to be infused and the rate of the infusion. 4. Your order is to give ½ maintenance fluids to your child who weighs 18 kg to be infused over 24 hours. Determine total volume to be infused (mL) and rate of infusion (mL/hr). PEDIATR IC FLUID
5. The child is receiving full maintenance fluids over 24 hours and their current weight this was 4 kg. Determine total volume to be infused (mL) and rate of infusion (mL/hr). 6. The child weight is 50 lbs. and is in renal failure. The provider orders D5 ½ NS to run at ½ maintenance for next 12 hours. Determine total volume to be infused (mL) and rate of infusion (mL/hr). 7. The nurse is caring for child diagnosed with diabetic ketoacidosis. The provider orders NS IV at 2 X (2 times) maintenance over 24 hours. The child weight is 150 lbs. Determine the total volume to be infused and what is the rate of infusion(mL/hr) that you will set pump at? 8. The child is in the ER for respiratory infection and current weight is 36 kg. The provider orders D5 1 /2 NS + 20 mEq KCL at ¾ maintenance over 24 hours. Determine the total volume (mL) to be infused and what is the rate of infusion (mL/hr). PEDIATR IC FLUID

9. The child has been admitted for submersion injury to the PICU with current weight 17 kg. The provider orders D5 NS for the child at ½ maintenance over next 12 hours. Determine the total volume (mL) and what is the rate you will set the pump (infusion rate)? 10. The child has been admitted for fracture of the left forearm and will have surgery in the am. The provider orders 1 ¼ maintenance fluids over next 12 hours. Current weight is 36 kg. Determine the total volume (mL) and what is the infusion rate (mL/hr). 11. The infant has been admitted to the PICU unit with RSV. The provider orders 1 ½ maintenance over the next 12 hours with D5 NS. The infant’s current weight is 12 kg. Determine the total volume (mL) and what is the infusion rate (mL/hr). 12. The child is admitted with current weight of 43 kg. The provider orders ¼ maintenance fluids to infuse over 24 hours. Determine the total volume (mL) and what is the infusion rate (mL/hr). PEDIATR IC FLUID
