ISBAR Communication Worksheet Client Name: Emma Date: 28 / 04 /22 Background A review of DCJ case files reveals that reports were made 2 years ago regarding Emma being exposed to 4 separate incidents of domestic violence, suspicion of child neglect, suspicion of parental substance misuse and self-injurious behaviour. The family is struggling following the loss of their father. They have a history of suspected abuse and neglect, and Julia exhibits signs of substance abuse. Emma, who used to be known to be very bright and bubbly, has been identified as withdrawn and sad. She occasionally attends school without food. Hannah has also become more difficult to manage. Two previous reports about family violence identified Peter as the perpetrator. He was also noted to have substance abuse issues. These reports also highlighted concerns about Emma's safety. The poor parental relationship between Julia and Emma was also noted in the report. In addition, the general practitioner of Hannah and Maggie noted that they had low weight and a skin condition. At the time of the report, the risk assessment noted that the family had a high likelihood of abuse and neglect. Unfortunately, due to various procedural errors, the case was closed. Situation Emma’s father Peter, died at the beginning of the year. This left behind Julia (mother) to look after Emma and the two older children, Hannah (15) and Maggie (9). Since the death of her father, Emma has noticeably been withdrawn from class and has repeatedly been arriving at school with no lunch, dirty clothes and is often picked up very late. Emma was referred to DCJ after not being picked up from school at all one day. This referral was made by Emma’s teacher at Northgate college, Amy. Introduction Hi, my name is Jennifer Magnisalis, and I am a student social worker at the University Of Newcastle. I am working with DCJ regarding Emma’s case. Emma is a 7-year-old girl who has been referred to us by her school, Northgate College.
Assessment - A new risk assessment should be conducted after an extensive interview with Recommendation My initial assessment of this family is that they are experiencing symptoms of grief and loss. However, based on all available information, they are not at risk of experiencing significant harm. Julia's financial concerns and the loss of her partner may be contributing to the moderate risk of neglect. It is not yet clear if the past traumas from abuse situations are affecting Julia and her daughters' responses to this event. There is also a lot of information about the family that is still unclear. This suggests that further investigation is needed to determine the level of neglect. It is also important to note that the family has a history of abuse and neglect.
