Share a brief summary of your understanding of the scenario.
John is a diabetic, who just recently moved to a new city. He went online to look for a new PCP
that appears to have a strong outcome in diabetes. John found one and scheduled an appointment
online, then he links his PHR to the new provider and requests that his previous PCP send over
his medical information to his new PCP. Everything was going well until John had an accident
one day that required a visit to the emergency department. He was admitted to the hospital and
outpatient physical therapy. All of his providers are members of the health information
organization (HIO) which enables each provider to have immediate access to all the information
needed to treat John throughout his care. John had consented to the sharing of such information
with all participants in the HIO. The hospital was able to save him money because they had
access to his previous labs and x-ray results, so they did not have to repeat any of that
information. This allowed the hospital to provide John with the proper care in a timely process
without having to wait on certain information like lab work. He was able to start his physical
therapy soon as well. His PCP monitored the impact of the accident on John’s diabetes during his
hospitalization and make adjustments when needed.
What is the name of the document that John's hometown physician can send to John's new

The name of the document that John’s hometown physician can send to John’s new primary care
physician is the electronic health records (EHR). The EHR is where all the health information
pertaining to the patient is stored and accessed by the healthcare professionals.
Why does John use a standalone personal health record (PHR) instead of one from his
provider?
John uses his own personal health records (PHR) instead of the one from his provider because it
includes more information from various providers he has seen over the years regarding his diet,
over-the-counter medications taken, and other information related to his diabetic treatment
regimen. It provides more information to his primary car physician, which enables the primary
care physician to retrieve information from his previous providers.
When John is hospitalized, what core EHR components enable medication allergy

The healthcare information systems have evolved and will continue too to help provide high-
quality care for individuals and communities. The healthcare information systems consist of
many people, technology software, and policies all working together for one goal, improving the
health and well-being of our nation. If you look back into how our healthcare used to be and how
it is now, you will see a huge difference. With the advancement of technology, the healthcare
systems have been able to utilize programs, hardware, etc. to be able to help provide better
quality care to people. A good example would be EHR (electronic health records) and CPOE
(computerized provider order entry) systems. The EHR system allows the healthcare staff to
view the patient’s information quicker and be able to see lab results, x-rays, medication, etc. The
CPOE is a very vital tool to have, including pharmacists. This system allows the healthcare staff
to input information in a patients EHR such as patient admissions, lab tests, x-rays, dietary and
nutrition, referrals, and medication orders. Pharmacists can view the medication orders and fill
these prescriptions quicker without having to wait or try to figure out what the doctor wrote. The
healthcare information systems are one big support group that helps each other provide the best
care that they can to an individual in need. Honestly, I had no idea of all the inner workings in
the healthcare systems, but I really do enjoy learning about it and getting a better understanding.
