Pharmacology Scenario 1: Junetta Cooper Guided Re:lection Questions Opening Questions How did the simulated experience of June6a Cooper’s case make you feel? I felt a little nervous not knowing how to handle the situation, but I followed the doctor's steps and instructions and I was able to manage well and correctly administer the meds. Tell me the ac=ons you felt went well within this scenario. The :irst thing I went well during the scenario was I remembered to ask her about her allergies. Next I remembered to reassess her pain after giving her both nitro and morphine in a timely manner. The :inal thing I remembered was the proper education on the medication I was dispensing to her. Reflec=ng on June6a Cooper’s case, were there any ac=ons you would do differently if you were to repeat this scenario? If so, how would your pa=ent care change? I would ask the patient sooner about their pain. I was more focused on my assessment when the pain crept up on us. The next thing I would do is read the orders a bit slower and take notes to re:lect on that makes things run by much smoother. Scenario Analysis Questions 1 PCC What priority problem(s) did you iden=fy for June6a Cooper? High blood pressure , high Heart Rate and heavy chest pain with cool clamy skin that was perspiring. PCC Iden=fy June6a Cooper’s risk factors specific to cardiovascular disease. S/PCC What poten=al problem could arise from sublingual nitroglycerin therapy, and what teaching should be included specific to this poten=al problem? I know that dizziness can be an issue with this medication. To avoid falling the patient will need to be properly educated on that aspect and how they can avoid an injury. Letting them know that they should rise slower and with someone in the room with them needs to be a priority. PCC/EBP Discuss the poten=al synergis=c effect(s) of June6a Cooper’s medica=on, and iden=fy the poten=al side effect this could cause. The potiential side effects are intense headaches. Orthostatic hypotension would also be another that was stated in the previous question. With the use of morphine they are more likely to experience the hypotension as well as decreased RR and nitroglycerin decreases preload and heart rate as well, inadvertently decreasing blood pressure. It is very important to monitor blood pressure as well as assess the patient for any dizziness, lightheadedness and orthostatic hypotension upon sitting and T&C What other interprofessional team members should be involved in June6a Cooper’s care? Case manager, social work, respiratory and cardiac... PT/Ot too Concluding Questions Describe how you would apply the knowledge and skills that you obtained in June6a Cooper’s case to an actual pa=ent care situa=on. I would apply the pharmacological information I regained about the side effects with the use of nitro and morphine together. I also learned more on how to care for a patient experiencing the acute angina. The last thing I took with me was a proper focused assessment when there is a speci:ic issue occurring.
