2 Goals Skills – Goal 1 - To become more familiar with the case management process and timeline for clients in the Hersong program. Goal 2 - Learn how to teach Purpose Group and begin facilitating the class beginning 9/18. Goal 3 - Enhance client interviewing skills to better assist in progress toward their goals. Goal 4 - Develop better client note taking skills. Knowledge – Goal 1 - Learn more about relapse prevention and group interaction for class. Goal 2 - Learn how to process SNAP and Medicaid applications with DCF quickly and efficiently. Goal 3 - Find out if a particular style of notes should be used in the case management documentation such as (SOAP or DAP).

3 Goal 4 - Engauge in learning how to apply EBP process and using that to apply appropriate interventions for clients and to analyze policy. Personal Development – Goal 1 - Read the staff book “Satisfy My Thirsty Soul” and find a new way to enhance my relationship with the Lord. Goal 2 - Attend weekly trainings with other RC’s to learn more about the process specific to Hersong. Goal 3 - Take time to participate in my intern and research courses which will develop skills needed for the workforce. Strategies Skills – Goal 1 a. Take time each day to review previous case notes and processes. b. Ask co-workers and supervisors to show me and teach me. c. Ask clients how it has been done previously.
