Liam is a 15-year-old male who resides mostly at his mothers Erica and spends alternative weekend and half of the school holidays with his father Micheal. Liam has a complex medical history. He had a right carpel/scaphoid fracture three years ago, in addition to asthma and juvenile idiopathic arthritis (JIA). Liam has lately been suffering from increased pain due to his JIA and has recently undergone increased asthma attacks/episodes. Some concerns of Liams mental health has also been raised, as his mother claims he is becoming more socially isolated and may be experiencing depressive episodes. This essay will focus on Liam’s primary and secondary healthcare providers, their roles in his treatment, the challenges Liam and his family must face, the referral process for seeing these specialists and the abilities required of the RN to organise and provide Liam with the best care. Patient Care Team Dr. Jones, a general practitioner (GP) and the registered nurse (RN) at the Youbeaut Clinic, are Liam's main medical point of contact. Liam's primary healthcare providers are the GP and RN; Liam and his family must schedule an appointment with the clinic in order to receive treatment, evaluation or to get referrals as necessary (Australian Government Department of Health and Aged Care, 2022). Liam's general medical care may include of regular check-ups, vaccinations, treatment, medications, minor injuries, and care coordination. Liam receives ongoing care from the Youbeaut Clinic's GP and RN, which means they keep a detailed medical document on him that contains information about his chronic diseases, treatment regimens, prior medical events, and goals allowing Liam’s team to have a proper understanding of his care which is able to be passed on to other medical professionals if required (RACGP, 2018). The primary care team is critical to the formation, implementation, and oversight of Liam's shared care plan. They accomplish this by scheduling and supervising appointments, facilitating good communication within the multidisciplinary team, informing them of neighbourhood organisations that may be able to assist Liam in meeting the targets of his care, and plan offering Liam and his family emotional and educational support (Australian Institute Of Health and Welfare, 2023). Nonetheless, seeing a GP does not require a referral. Appointments are required for most general practitioners. If you arrive without an appointment, the general practitioner may not be able to see you or you may have to wait a long time in order to be seen (myGov, 2024). A paediatrician, a secondary healthcare practitioner, treats Liam's asthma and is critical in ensuring that it does not interfere with his daily activities. The four basic aims of care for paediatric AAE management are early and objective assessment of severity, timely and efficient medicinal measures to minimise respiratory, suitable patient disposition, and safe
discharge planning (Leung, 2021). Liam's asthma control needs to be monitored on a quarterly basis. The Asthma Action Plan must be reviewed and adjusted, and Liam's medication or dose may need to be changed due to the dust storms in his hometown with has caused him to have more asthma attacks (Asthma Australia, 2021). A referral is necessary in order to schedule an appointment with a paediatrician, who specialises in treating infants, kids, and teenagers in order to keep costs as low as possible. A general practitioner (GP) can provide a referral and provide a thorough medical history in addition to the referral to the specialist along with checking to see if Medicare or private health can rebate any of the costs (The Royal Children's Hospital Melbourne, 2018). Additionally, the paediatrician gives Liam and his family thorough instruction and assistance in managing their asthma. This instruction can involve recognising symptoms, knowing asthma triggers, and learning the proper technique for using an inhaler (National Asthma Council Australia, 2019). Liam's physiotherapist, whom he visits once every two weeks, is another member of his secondary health care team. Providing physiotherapy services specifically tailored to address Liam's pain and movement difficulties due to his JIA, making the physiotherapist a crucial role in his healthcare team (Health Direct, 2018). The physiotherapist's main duties include determining which joints or body parts are affected by JIA-related pain, tailoring exercise programs to help improve movement with JIA, concentrating on ways to reduce pain and improve Liam's overall pain control and evaluating Liam's pain threshold in conjunction with
other members of the multidisciplinary team (Health Direct, 2018). In order to see a physiotherapist a referral is needed. In order to receive financial support in regards to physiotherapy sessions from Medicare, Liam will need to have his doctor put it on his shared care plan and refer him to the Management Plan and Chronic Disease Management Plan, where if approved Medicare will finically help with 85% of the overall cost for 5 sessions per year (Australian Government, n.d.). Liam's JIA is overseen by his paediatric rheumatologist, which needs a referral from another doctor. a paediatrician with expertise in rheumatic diseases, which include diseases that impact muscles, joints, and connective tissues, including juvenile arthritis (The Royal Children's Hospital, 2020). In Liam's instance, the paediatric rheumatologist's job is to work with his primary care team to create a thorough treatment plan that is customised to meet Liam's needs and objectives (The Royal Children's Hospital, 2020). In addition, the paediatric rheumatologist prescribes and oversees the effectiveness of the medications used to treat Liam's JIA, which may include breakthrough pain relievers such as paracetamol and nonsteroidal anti-inflammatories such as Naproxen (Australian Rheumatology Association , 2023).
