I am addressing your concerns regarding our center's proposed quality improvement initiatives. After thoroughly analyzing the center's organizational metrics, I have identified several areas for improvement. These initiatives are based on evidence- based literature and aim to reduce the risk of patient falls in our facility. Prompt 3. Falls by AHRQ Firstly, I would like to highlight the significance of patient falls in our center. Over the past 30 days, we have experienced five patient falls, significantly higher than the national average at a rate of 26.74 falls per 1,000 patient days. “ Each year, roughly 700,000 to 1 million patient falls occur in U.S. hospitals, resulting in around 250,000 injuries and up to 11,000 deaths. About 2% of hospitalized patients fall at least once. Approximately one in four falls result in injury, with about 10% resulting in serious injury. These incidents have raised concerns about patient safety and may require immediate action.” ( LeLaurin, et al, 2019) To support the need for change, I have provided you with a comprehensive analysis of the organizational metrics related to the fall incidents. These metrics include patient demographics, risk factors, and incidence patterns. By analyzing this data, we can identify patterns and trends that may contribute to the occurrence of falls. This data and evidence-based literature allow us to develop targeted interventions to prevent future incidents. 1. Elements of evidence-based literature

Data collection: Reliable data collection is essential for evidence-based reports. It is necessary to determine if the study utilized appropriate methodologies, such as random sampling, to minimize biases and ensure the findings are generalizable. 3. Credibility of the sources: Assessing the credibility of the report's source involves considering factors such as the author's qualifications and expertise, the rigor of the methodology, and conflicts of interest. 4. Validity of the report's conclusions: To assess the validity of the report's findings, the strength of the evidence, the validity and reliability of the methods used, and the presence of confounding variables must be considered. 3.
