The five social determinants of health are: Economic Stability Concerns about the cost of care, a lack of accessible healthcare or insurance, and a living wage have been highlighted as the top economic issues affecting health, emphasizing economic stability as a significant social determinant of heath. Health Care Access Barriers to accessing mental health services, such as the cost of care and a shortage of healthcare providers, highlight the relevance of healthcare access as a social determinant of health. Built Environment Factors leading to chronic disease, such as the presence of food deserts, access to physical activity, and environmental health concerns, highlight the role of the built environment in impacting health outcomes. Social and community context Minority population disparities, mental health provider shortages, and problems navigating the healthcare system highlight the importance of strong social and community support in addressing mental health and overall health needs. Education, Access, and Quality Providers' lack of cultural competence underscores the significance of cultural competency as a social determinant that affects healthcare access and outcomes. Explain how secondary data was used to help create the health needs assessment. Secondary data is pre-existing data used for record-keeping by large government institutions, healthcare facilities, and other organizations (Benedictine University Library, 2023). Quantitative data was collected from multiple reputable sources such as the Behavioral Risk Factor Surveillance System (BRFSS), County Health Rankings and Roadmaps, Florida Charts, Florida Department of Health, Local Community Health Assessments, Hospital Utilization Reports, U.S. Census, and the Youth Risk Behavior Surveillance System (CHNA, 2021). These comprehensive data sources thoroughly examine health-related variables such as behavioral risk factors, healthcare utilization patterns, and socio-demographic indicators. The collected data spans several areas: health status, epidemiologic investigation, healthcare usage, service access, and self-reported analysis
(CHNA, 2021). By adopting this inclusive methodology, we can attain a holistic comprehension of the population's physical, mental, environmental, and social well-being. According to CNHA (2021), Healthy People 2030's universal measures serve as the framework for analyzing the overall health of the U.S. population. Life expectancy (L.E.) allows for accurate and direct comparisons across time and different regions with varying population characteristics (Boothe et al., 2018). This critical metric not only gives an overview of overall well-being but also allows for relevant studies of patterns and disparities, contributing to a comprehensive understanding of community health dynamics. It is an essential component of the Health Needs Assessment (HNA) that can be influenced by various factors, and secondary data is valuable for understanding and interpreting those factors. The identified factors include high infant mortality rates, increased instances of substance overdose or suicide, and barriers to high-quality healthcare (CNHA, 2021). Beyond life expectancy, the study examines mortality and morbidity rates, focusing on the top five major causes of death in the service areas: cancer, chronic lower respiratory disease (CLRD), heart disease, unintentional injuries, and stroke (CNHA, 2021). Changes in mortality rates over time, such as the 2.7% increase in deaths in Duval County from 2017 to 2019, reveal trends and potential emerging health risks.
