In your own words, explain the need for standards for sharing healthcare data between different information systems/organizations. There are many standards healthcare systems need to follow when sharing data. If there were not any standards there would be unnecessary records sent to facilities that do not need the patient’s information. 2. Explain the impact of data standards on the quality and safety of patient care. Give a specific example. Data standards on quality and safety is how data is collected and stored in the system. An example would be if a patient was receiving medication and has an allergy to a certain medication that could be given the alert would pop up and notify the staff. 3. Describe a clinical vocabulary and its function in an EHR. UMLS stands unified medical language system and uses terminology and code standards to classify resources to link codes and terms between any physician and insurance company. https://www.nlm.nih.gov/research/umls/index.html Exercise 2: EHR and communication between providers and level of care (4 points) 1. In your own words, explain the relationships between the continuity of care record, continuity of care document and the clinical document architecture. The relationship between these three topics is when a patient is going to another facility records need to be sent. All necessary information must be sent in order for the physician to care for the patient. Using all three will eliminate errors and make sure all areas are met when continuing care for a patient. 1

Explain the reason why this functionality is important to include when evaluating an EHR for use by a healthcare provider. This functionality is important as it has templates that allow certain records to be sent depending on what the needs are for the receiving facility. In some EHRs it is very important to have this feature so that employees can send the correct records that are needed by the physician. It will also give the physician a chance to view what you want and to make sure they have everything documented that is needed. Exercise 3: Meaningful Use (10 points) The HIM field is abuzz with talk of the meaningful use incentive program for electronic health records. HIM professionals can play a significant role in facilitating hospitals and physicians meeting and attesting to compliance with the requirements. 1. What is the relationship between Meaningful Use and MIPS? (2 points) MIPS uses meaningful use to put data together and to provide incentives to providers. It is used to improve patient quality and safety and only relaying what is needed for the research. 2. List and describe two criteria that are required for meaningful use. (4 points) 3. For one of the requirements listed in 2, describe two steps that the HIM professional should take to make the hospital or physician (state which one) compliant with this requirement. (4 points) Exercise 4: Effects of EHR on HIM functions (4 points) 1. Describe the process used to correct errors in an electronic health record. Does the process differ for different types of errors? Explain. When correcting an error in the EHR you would use an amendment that is when you want to alter any point of the alter anything in the health record. There is an addendum is when you are wanting to add anything the record. Correction is a change in the information to clarify. There is deletion which permanently eliminates information. Explain the use of metadata in the error correction process for electronic data. Exercise 5: Thought Questions (12 points) 1. Is a medical record that includes only scanned digital documents an EHR?
