A1. Three duplicate patients I’ve found within the Master Patient Index are Lillian Nolan, Sean Young, and Ian Clarkson. A2. There are many errors within the Mater Patient Index. Three of which I have found are: Janice “Jan” Powells birthdate is input incorrectly. There are two accounts with the same SSN but the birthdates do not match. Yvonne Edmunds SSN was not input correctly on either account which is causing duplicate profiles. David McGrath has three profiles simply because the name was entered incorrectly each time. B1. The Sample Data Dictionary is missing correct formatting, the information should be sorted by separate columns at the top, labelled for each identifier such as DOB, SSN, account number etc. The sample is missing the patient’s first and last name as well as suffix. The column titles should be Last Name, Suffix, First Name, DOB, SSN, Account Number, and Medical Record Number.
