. Students must pass with a grade of B or 80% (94 points out of
118 available points) in order to continue in the clinical setting.

SKIN, HAIR and NAILS: (2 points)
Inspect skin, hair and nails (including capillary refill) throughout entire exam
HEAD (16 points)
Inspect and palpates head and scalp
Inspect face at rest and with movement (CN VII)
Palpate sinus areas
Test forehead, cheeks, jaw for pain and light tough (CN V)
Palpate temporomandibular joints
Test jaw clenching (CN V)
EAR (10 points)
Inspect and palpate auricle
Test hearing with whisper test (CN VIII)
Perform Weber test for lateralization (CN VIII)
Perform Rinne test for air and bone conduction (CN VIII)
Inspect ear canals and tympanic membrane
NOSE & MOUTH (12 points)
Inspect nose and olfactory sense (CN I)
Inspect the entire mouth for lesions. Note dentition.
Examine Wharton’s and Stensen’s ducts
Test tongue movement (CN XII)
Test swallow, gag, palate and uvula motion (CN IX, X)
Assesses taste (anterior CN VII, posterior IX)
EYE (12 points)
Inspect external ocular structures, including lacrimal apparatus, conjunctiva, sclera, cornea, iris
Observe pupillary response to light (direct and consensual) and accommodation (CN III)
Test visual acuity (CN II)
Assess visual fields by confrontation (CN II)
Evaluate external ocular muscle function, eye alignment (CN III, IV, VI) said 3, 4 and 6
Inspect the lens and retina with ophthalmoscope
NECK (10 points)
Palpate and names lymph nodes (preauricular, postauricular, occipital, tonsillar, submental,
submandibular, superficial cervical, deep cervical, and supraclavicular)
Inspect and palpate thyroid with swallowing
Inspect jugular neck veins for distention with HOB elevated 30 degrees
Palpate and auscultate carotid arteries
Test shoulder shrug and head strength (CN XI)
CHEST (14 points)
Inspect costal angle and AP/Lateral ratio
Palpate for symmetrical tactile fremitus
Percuss the chest systematically for resonance
Auscultate lung fields
Palpate cardiac for heaves, thrills, location of PMI
Auscultate all 4 areas for cardiac sounds
Palpates arterial pulses
ABDOMEN (18 points)
Inspect abdomen for pulsations or peristaltic waves
Auscultate in all 4 quadrants for bowel sounds
Auscultate aortic, iliac, renal and femoral arteries for bruits
Percuss the abdomen in 4 quadrants
Percuss the liver to determine size
Percuss for CVA tenderness (lateral towards back)
Percuss spine
Perform superficial and deep palpation including liver and spleen
Palpate for inguinal lymph nodes
MUSCULOSKELETAL (8 points)
Inspect for joint symmetry, alignment, bony deformities and swelling
Determines range of motion of 1. upper extremities, 2. lower extremities, 3. spine, and 4. neck
Touch toes, assess spine
Tests for strength of upper and lower extremities
NEUROLOGICAL (16 points)
Test DTR’s (biceps, triceps, brachioradialis, patellar, Achilles)
Inspect gait
Test motor function by having patient walk on toes and heels, hop in place, perform shallow knee bends
Assesses cerebellar function by having patient perform tandem walking, and Romberg
Assess coordination by having patient perform rapid alternating movements, point-to-point testing,
heel-shin
Assess pain, light touch and vibratory sensation
Assess stereognosis, graphesthesia and proprioception
Assess cerebral function/mental status
