The correct answer is all of the above. Equipment can be locked out. It can simultaneously be tagged out as well. When equipment is not really capable of being locked out, or when portions or pieces of equipment cannot be locked out, it is acceptable to tag out. Sometimes heavy equipment may be in the field and being serviced or repaired and in a manner where the machine may need to be started, but portions of its movement be locked, blocked/cribbed and in this situation the machine will be tagged. In some situations such as articulated steered equipment, red lock pins/safety arms are locked in place with pins that prevent movement. Some equipment may need its blade or bucket cribbed controls tagged, but not locked out completely for troubleshooting. Some equipment will be removed from the site or parked/staged
in maintenance areas as well. This is another method of control.
Now key control is a method of control as well. The key can be locked in a lock box, or controlled by the mechanic/operator performing the servicing or troubleshooting in order to prevent accident start up or use.

Both A and B are qualifiers. This goes along with the definition of a "qualified" person per OSHA usage. It is important to note that training, is formal classroom lectures/interaction which can include videos, digital training, and also evaluations and observations are "training" per OSHA. (remember from OSH 262). But it also takes documented successful experiences to prove qualified status. This is experience. You can track assignments on projects to specific
equipment, document evals and observations as experience, and/or document hours operating.
Typically a good program begins with classroom education/training on the equipment which may include demonstrations. It progresses to practical exercises that mimic conditions and actions typical of the equipment and work. Once proficient practical exercises are completed for
evaluation, the operator will operate in real conditions under the supervision of a more experienced person (mentor or journeyman operator). Observations or evaluations can and will take place until the person reaches a level of competency that qualifies them to be a mentor/trainer. Even then reviews of new techniques, accessories, equipment, or even evals should be take place.
