Scenario:
A mother calls Dr. Walden’s office and asks to speak to the physician. You tell her that the physician is not available. She then proceeds to demand that you tell her what is wrong with her daughter who had seen the doctor a month ago. Her daughter is 17 years old and still lives at home. She is on her parent’s health insurance and mom just received a bill indicating that her daughter saw the doctor and had the visit submitted to the insurance company.
Per the chart, you see that the patient saw the Dr. for a pregnancy exam. As the medical assistant, you tell the mom that you cannot give out patient information. She continues to demand information stating that it is her right to know since her daughter is still living at home and on her parent’s insurance. She states that she will not hang up until you give her the information.
1. What is your priority/responsibility to your patient

My priority and responsibility to the patient is to maintain her confidentiality by not giving
out any of her personal information because the treatment she received is confidential and by law, it doesn’t have to be disclosed in this case. 2. What is your responsibility to the mother? (X.P.2.a-b)
Although the mother learned about the treatment through an insurance bill, I still will have to explain why I am not able to share any further information to her due to patient privacy and HIPPA laws. It is my responsibility to make sure she understands why I can’t share any details. 3. What cultural or diverse issues may be present in this situation?

The mother stated that it is her right to know because her daughter still lives at home. Many parents believe that because they still provide care for the kids and the child is under 18, they have a right to know any and everything that goes on with them despite legal laws as seen in this situation. 4. What would have been a better way of handling this situation from the start, so it didn’t get to the point of the parent calling about a bill? When the daughter initially was seen by the doctor for a pregnancy exam, she should’ve
been informed that her pregnancy testing and prenatal care can be kept confidential but
because she is on her mother’s insurance, her mother may learn of the treatment through insurance statement’s. By giving the patient this information, she’ll be more willing to discuss this with her mother beforehand. She could also be given other options for free/public clinics that provide pregnancy and prenatal care where insurance isn’t required.
