Purpose
This Observation Form outlines the specific criteria that the candidate must demonstrate/perform while
completing Workplace Assessment Task 1
.
This form is to be completed by the candidate’s assessor to document their observation of the candidate’s
performance in Workplace Assessment Task 1
.
Task Overview For this task, the candidate is required to work with at least two
other personnel to identify and document
hazards in their workplace, while being observed by the assessor.
For this task, the candidate is required to complete the following while being observed by the assessor:
Work with the two other workplace personnel to identify workplace hazards including:
o
Conducting a safety inspection in the workplace
o
Reviewing copies of current WHS reports
Each workplace personnel can participate in either one or both of these tasks.
Store hazard identification records following organisational procedures, including:
o
The Hazard Identification Checklist
o
At least four Hazard Report Forms - one for each required area of hazard identification In this task, the candidate will be assessed on their:
Practical knowledge of workplace hazards
Practical skills relevant to identifying and documenting workplace hazards
