Economic stability is vital to health. People need to eat, have a place to rest their heads and be able to seek out medical attention should the need arise. “Economic stability is defined as the ability to access resources that is essential to one’s life and well-being” (Centene Corporation/ CWLA, 2021). 1 in 10 people, in the US live in poverty and 1 in 3 meet criteria for economic instability; meaning they’re incomes are 200% below federal poverty level (Centene Corporation/ CWLA, 2021; US Department of Health and Human Services, 2020). Disparities in social determinants exist for a number of health-related indicators such as infant mortality, life expectancy, access to healthcare services, and mortality from leading causes of death (US Department of Health and Human Services, 2020). Reducing the portion of people living in poverty SDOH-01 has had little of no detectable change. As mentioned above, poverty is a key component of economic stability and strongly increases risk of poor health (Centene Corporation/ CWLA, 2021; US Department of Health and Human Services, 2020). When assessing a patient’s economic stability status, I feel there is a certain nuance to asking difficult questions. They sought help. We must make them feel like they are seen and heard and aren’t being judged. Judgementalism undermines quality healthcare too often and leads to negative outcomes for the patient (Weiner, 2021). Questions to include in a comprehensive general assessment include, “Tell me about where you live. What kind of home do you live in?” “Tell me about your home life and support structure. Do you live with anyone? How many people live in the home?” “Are you able to buy food and medicine without issue?” Asking what kind of home
they live in will provide insight of HOW they live- are they living in their car? A weekly rent motel? A 5 bedroom, 2 story, house? Asking about their home life and support structure gives us a glimpse into if they have a support structure in place or if they have multiple roommates, none of which they trust. Asking about the ability to afford food and medicine is crucial. If they can’t afford food, the patient likely can’t afford to be compliant on a prescription medication regime. The practitioner needs to take note of the patient’s financial status and act accordingly. Offering resources such as HUD or food stamps. These are well known resources to many patients. HUD offers rental help, offering reduced rents to low-income households, public housing, housing choice vouchers (section 8), and offers resources for eviction prevention, rural housing options, and other sites that offer affordable housing in Florida (U.S. Department of Housing and Urban Development, ND). Florida also has a program called Supplemental Nutrition Assistance Program (SNAP). Advertised through the Florida Department of Children and Families and federally supported, SNAP provides grocery support to low-income families, people living with disabilities, fixed income families and other individuals with low income (Florida Department of Children and Families, ND).
