Knee Pain • Etiologies o Acute or chronic o Traumatic or Atraumatic • Location of pain is helpful • Some differentials o Baker’s cyst – popliteal cysts seen with RA, OA, internal derangements of the knee; can rupture causing pain, erythema, swelling o Patellar tendonitis: Often will not present unless rupture has occurred, With rupture get large effusion and limited ROM (extension) o Ligament sprain: ACL, MCL • Anterior Cruciate Ligament (ACL) Tear: h/o injury with popping, swelling and giving way of knee • Medial Collateral Ligament (MCL) Tear: h/o injury with pain, joint instability, force against lateral knee (common in football) o OA, RA o Referred pain from back, SI joint or hip History o age o location, onset, duration, and quality of pain mechanical or systemic symptoms o history of swelling o description of any precipitating trauma o pertinent previous medical or surgical procedures Consider urgent referral: severe pain, immediate swelling, and instability or inability to bear weight in association with acute trauma or signs of infection
(fever, swelling, and erythema with limited range of motion)
