Sherin Cerdon Lab assignment 1. Mr. L is on contact precautions for MRSA. He requires assistance to set up his meal tray. Describe the PPE required to enter Mr. L’s room and explain the doffing procedure? Mr. L is on contact precaution so the PPE that the CCA should wear are gloves and gown. In removing the PPE, the CCA will remove the gloves first (discard the gloves) make sure gloves wouldn’t touch the skin (glove-to-glove), then followed by the gown. To remove the gown, untie the neck string first then the waist. Allow the gown to fall away from the shoulders. Once the gown is off the shoulders, pull one arm at a time from sleeves so that the gown arms bundled at the wrist. Then, roll the exposed or contaminated outside of the gown inward and away from the body. Then dispose it immediately. Hand hygiene should perform in each step. 2. Mrs. S needs her bed made. She will be returning to bed after using the toilet. What type of bed would you make for Mrs. S? Describe the steps to making it. The type of bed for Mrs. S is an open bed as a result of she will returning to bed soon after using the toilet. Steps in making an open bed: Make sure you have all the required linens in making bed and placed it in a clean surface near you and the hamper/ laundry bag too. Raise the bed to a comfortable height or waist level. (Assume all the soiled linens are gathered and put away in laundry bag.) If you are alone, work on one side of the bed at a time. Bed rails should be lowered on the side that you are working. First thing to do is hand hygiene. Then fold out the bottom linen or fitted sheet in half lengthwise of the bed and tuck into corners under the mattress the side that you are working. Fanfold the other half and leave it at the center of the bed. Next is place the bed pan (if needed) in the center of the bed. Open it on the side that you are working then fanfold it to the other side of the bed. Then place the top sheet, same thing goes. And then go to the opposite side of the bed. Unfold the remaining half. Add bedspread, then pillow case. Make sure the sheets are smooth and no crease. 3. Mrs. P is unable to feed herself and has dysphagia. She is on a soft diet. Describe the 4 steps the CCA should take when feeding someone with dysphagia. CCA should avoid Mrs. P feeding in bed, it should be in a chair in an upright position so the food will not go directly to the airway. Talking while eating should also be avoided and eat slowly in small bites. Lower the chin to chest before swallowing. If happens that Mrs. P is in a condition where she can only stay in bed, the CCA may adjust the bed to high- fowler position for Mrs. P to be in upright position and her spine is straight.
4. Mr. B needs to be repositioned onto his back. Describe how the CCA should support his limbs and ensure proper body alignment. For Mr. B lying on his back, the CCA must check the following: Place pillow under the head. You can also put rolled towel under the neck for neck’s support and neutral alignment. Place rolled towel under the waist for support. Arms should either be on the side or on his chest. Support under his shoulders, elbows, and arms with pillow. Make sure Mr. B is in straight position and not twisted. Legs should be slightly apart and feet is relaxed. Place pillow under the leg or under the knees for short period of time with the permission from supervisor because this may lead to contractures of the knees. Footboard at the end of the bed to give support and relieves the pressure of the bedding for feet and ankles. Pressure points should be check and beddings has no crease to avoid discomfort.
