What Considerations/Actions Should the Nurse Make Regarding the Client's Refusal to
Restart His Blood Pressure Medication?
Various factors, such as the prescription drugs' smell, taste, or color, can cause patients to
refuse them. Patients occasionally decline medication due to side effects or anticipated adverse
experiences. My first duty is to see that Mr. B understandably receives information about the
potential repercussions of his choice. Additionally, I'll start a friendship because it can ease the
tension and anxiety of taking medicine. I would look into less complex drug options if he refused
the prescription. Nurses may suggest changing to liquid formulations or crushing large tablets
into tiny pieces since clients may reject tablets that are excessively big or difficult to chew.
What Considerations/Actions Would Have Helped the Healthcare Team to Prevent the
Client's Fall?
The majority of the time, accidents and falls are not just random occurrences. The
medical staff should design an appropriate workout regimen for Mr. B. In addition to
strengthening and flexing the client's joints, tendons, and ligaments, regular exercise will
enhance Mr. B's muscles. The medical staff should also set up a fall-proof at Mr. B's house. Due
to the patient's poor visibility, the team should ensure sufficient illumination with light switches
on either end of a long hall. They should also place light switches and nightlights close to his
bed.
