How does our bureaucracy intersect with healthcare? The Indian Health Service that works under the Department of Health and Human Services, is the steppingstone in addressing the many health needs of American Indians and Alaska Natives. One program in particular that I read about is the Indian Health Care Improvement Act. This was a law that was brought up in 1976 but was not made permanent until 2010. The main purpose for this was to provide more care and make care more accessible to urban indians. It was also to implement the federal responsibility for the care and education of the Indian people by improving services and facilities of federal indian health programs. The main goals of the IHCIA were a little extreme but also very much needed. One main goal was to eliminate the health inconsistencies among Native American populations as well as elevate the overall health status of American Indians and Alaska Natives. Along with this mission was the vision of implementing comprehensive yet culturally sensitive health care services that have preventive care, behavioral health services,and necessary medical treatments. All in all the main purpose of this was to foster a more holistic approach instead of addressing an immediate health concern. With going in the direction of a more holistic approach to medicine it gives a more unique cultural and historical context of these many communities. An oversight that this program has is the ongoing subject. There have been many steps in the right direction for certain health indicators and systemic challenges, but there are definitely some flaws that have some struggles. A few things that I saw while doing some research were workforce shortages, underfunding, and jurisdictional issues. Geographical barriers further exacerbate these challenges especially in remote and underserved seas.
When discussing the budget of this act, as of 2023 the budget of this act was $7.1 billion with the prediction of the budget increasing to $53.85 billion for 2025. When it comes to the effectiveness of this act, according to Michigan State University, it does a pretty good job of increasing revenue for the Indian health system, allows more self determination rights for those in health care regulations and decision making, and most importantly it improves the quality of life by increasing the number of programs that offer substance abuse services along with behavioral health. To bolster the effectiveness and efficiency of this act there are a few things to look at. Increased funding would facilitate the expansion of services, improvement of the foundation, and most importantly the hiring of skilled healthcare workers. The need of a complex healthcare system is vital and is a necessity to attract and keep talent in different environments, especially remote ones with telehealth technologies becoming more and more popular. In conclusion the Indian Health Care Improvement Act, plays a vital role in addressing and drawing attention to health challenges faced by Native American populations. Although a lot of progress has been made the right amount of effort is needed to increase spending, strengthen partnerships, and integrate innovative strategies. By putting cultural competence, preventative care, and community gathering at the forefront, the Indian Health Service cna make a course toward a future where the well being of Americans and Indians were uplifted reflecting the noble goals of the Indian Health Care Association.
