State your proposed EB practice
change, 15 points
Implementation of an integrative complementary and
alternative medicine program for cancer patients with
advanced disease progression that would be seen as a
viable alternative to opioid analgesics for oncologic
pain. Participants will be given opportunities to visit
with acupuncturists, massage therapists, and yoga
instructors. Follow ups with primary care oncologists
would occur monthly and include pain assessment
questionnaires and survey regarding the effectiveness
of CAM intervention. After 6 months other CAM
providers would be included pending the results of
the pilot study.

the change process in your
institution (or where the change will
be implemented). Include the
organization's culture, anticipated

Some potential barriers to having a fully integrated
CAM medicine program would include skepticism on
behalf of the providers or patients who are not fully
aware of the benefits of alternative therapies. An
institutional pilot study showing the effectiveness of
CAM in the patient population would need to be put
together and be put forth for IRB approval. Other
barriers include funding and seeking active
participants to participate in the pilot study. From an
institutional standpoint volunteers and staff members
would need additional training in CAM specialties
(employment of massage therapists, yoga instructors,
acupuncturists, etc.) requiring additional funding if
grants could not be obtained. The culture of the
organization is very focused on new groundbreaking
research and prides itself on new innovations. The
leadership role needed to properly implement this
pilot program would include a research studies
coordinators or a research nurse to organize and
obtain the needed approvals to get the program up
and running.
Plans to overcome barriers

To overcome these barriers educational sessions for providers
and literature regarding the pilot program of CAM would be
provided during regularly scheduled research meetings.
Marketing flyers promoting the CAM program would be
placed in waiting rooms, common areas, individual exam
rooms and other common patient areas deemed acceptable.
The research department would oversee spearheading the
IRB approval process and setting up the calendar/schedule
for patients and CAM providers. Special requests for
budgetary funds and funding for contract CAM providers-
yoga instructors, acupuncturists, and massage therapist to
compensated appropriately and facilities to be used.
Who are the colleagues you will
include to facilitate change?

The institution is fortunate to have a research
department that would oversee the pilot program and
actively recruit patients, collect data and
questionnaires, and follow up with the patients either
in person or via telephone calls. Medical providers
would also be vital in implementing change by allowing
this option and suggesting the CAM program to their
patients when the subject of pain comes up. An
interdisciplinary approach would be needed in order to
come up with the best outcomes for patients.
Identify the stakeholders for the
change in practice you have chosen
for your EB project. 20 points
The main stakeholders for this change in practice would
be the patients who would benefit from alternative
therapies for oncological pain. Other stakeholder that
would also be affected includes the family members or
caregivers of the patients in addition to doctors, nurses,
CAM practitioners (massage therapists, fitness and
personal trainers, acupuncturists) and other member of
the medical team (PT, OT, Social Work, Case managers)
and to a greater extent the hospital and cancer research.
