Consider how the setting in which you see clients may impact your recommendations for medications, treatments, or follow-up care. The setting will greatly impact how a patient is treated. In many settings, imaging and blood work are not available so these would have to be done at another place. So, the patient would need to go get the testing done and treatment would be for the symptoms. The patient would then return for another appointment to go over the results and receive more adequate treatment.
How would your recommendations have changed for this client if she was underinsured? If this patient was underinsured, an MRI would likely not be covered. Instead of imaging, diagnosis and recommendations would have to be based off the physical findings and patient history. Education would be imperative to ensure prompt return if any ‘red flag’ symptoms present. Treatment would entail bedrest and over the counter pain relievers.
What about if the encounter had occurred at a mobile clinic for unhoused clients?
If this encounter happened at a mobile clinic for unhoused clients, it would be very similar to the underinsured. Imaging would be forgone and treatment would be mostly education. Social services should be consulted to
find a place to stay for a few days in order to rest properly.

The patient is a 35 year -old female with AIDS brought to the emergency room with a fever of 39oC and a three month history of copious diarrhea.
On physical exam the patient is a well-developed, thin female in moderate distress. Vital signs-(supine) blood pressure 100/60, pulse 100 and (standing) blood pressure 80/40, pulse 125, respirations 18 and she was afebrile. HEENT exam was normal. Cardiac exam demonstrated an S1 and S2 without S3, S4 or murmur. Lungs were clear to auscultation and percussion. The abdomen was supple and minimally tender to palpation. A 22-year old woman in her 2nd pregnancy has arrived in the labour ward at 38 weeks 3 days. She had a normal delivery 18 months ago. This pregnancy has been complicated by persistent vomiting until 20 weeks and more recently by anemia. She reports contractions commencing approximately 4 hours ago. She took paracetamol at home and tried to relieve the pain with a bath, but now she feels she cannot cope with the pain.
She had a show 2 days ago but has had no bleeding since then and has not noticed any vaginal leak. She has felt the baby moving normally all day.
Physical Examination:
BP is 110/58 mmhg, heart rate is 98/min. The presentation is cephalic with 2/5 palpable abdominally. Uterine contractions are palpable and the uterus is non-irritable.
