Post a completed logic model (in table format) for a practitioner-level intervention based on the problems and needs you identified last week. Be sure to link the areas across the table. In the table, describe the types of problems, the client needs the underlying causes of problems, and unmet needs. Then describe interventions that would lead to a change in the presenting conditions. Identify the short- and long-term outcomes that would represent an improved condition. Therapy and counseling are the first steps to a brighter future. Tiffani explains that a young man named Donald approached her and inquired about her whereabouts at such a late hour.
Tiffani continued to maintain a close relationship with Donald because she felt comfortable and secure there. Tiffani has a history of eluding capture and has been taken into custody for prostitution a few times in the past two years. Tiffani has not attended school since she ran away from home two years ago. Tiffani has burn scars all over her body, has had STI treatment, and is taking medicines for a kidney infection right now. Tiffani admitted that her uncle had previously molested her. She can't recall anything since she was fourteen years old. Tiffani reveals that her parents used drugs and alcohol but were not present. She decided to leave her house at this point because she felt unsafe. However, if Tiffani didn't follow his instructions, Donald had been violent to her. She does state that she misses her mother and sister, though. There was once a tight relationship between Tiffani and her younger sister. Tiffani has demonstrated her resiliency by learning how to endure the toxic relationship she has been a part of. She feels a duty to defend her sister and is willing to give her life to do so.
