To alleviate his pain, he turned to alcohol and eventually other substances to self-medicate as his PTSD and anxiety continued to worsen. This led to significant losses in relationships, employment, health, and freedom. Fortunately, there are substance abuse treatment options available in the correctional setting, such as mindfulness-based stress reduction and Vipassana meditation, which could allow Angel to maintain his sobriety and avoid re-offending. Diagnostic Impressions Based on Angel's Post Traumatic Stress Disorder and cocaine and alcohol dependence, the clinician recommends Trauma-focused Cognitive Behavioral Therapy. Additionally, Angel may benefit from Motivational Interviewing and Cognitive Behavioral Therapy, as well as medication like Disulfiram to
support abstinence from alcohol. Mindfulness-Based Stress Reduction sessions could also be helpful for Angel's overall well-being. (Edo, Van Gordon, & Griffiths, 2014) Alcohol Use Depression Anxiety PTSD Substance Abuse Goal Lists Angel has set some clear and achievable goals to improve his health and well-being. He is determined to abstain from alcohol for three consecutive days by July 5 th , 2024, as part of his commitment to maintaining a healthy lifestyle. Additionally, he has planned to follow his medication schedule without missing a single dose for two weeks before May 28 th , 2024, to effectively manage his dual diagnosis. Lastly, he is excited to attend mindfulness-based stress reduction meetings three times a week for six weeks, as recommended by (Edo, Van Gordon, and Griffiths in 2014). Angel is confident that he will successfully achieve these goals and improve his overall health and well-being. Objectives The primary issue that Angel needs to address is his alcohol consumption. The objective is to completely abstain from drinking alcohol. To achieve this objective, Angel needs to identify the triggers that can lead to relapse. This objective was established on April 12 th , 2024, and the target completion date is May 15 th , 2024. Another objective is to expand his social support by finding substance-free activities. The objective was established on April 12 th , 2024, and the target completion date is May 30 th , 2024. The clinician will help Angel to identify triggers, educate him about high-risk situations, and address any potential ambivalence by using Cognitive Behavioral Therapy and Motivational Interviewing. The clinician will also help Angel expand his social support network by finding non-using friends, suggesting social activities, and educating him about the benefits of a healthier lifestyle. This will be done weekly in a 60-minute individual session for three months. Angel's second problem is that he is at risk of losing his family if he continues to drink. The goal is to begin to resolve family conflicts while maintaining a substance-free lifestyle. The first objective is to identify high-stress situations with the family that may trigger drinking. This objective was established on April 12 th , 2024, and the target completion date is May 30 th , 2024. The second objective is to find three non-using friends to socialize with. This objective was established on April 12 th , 2024, and the target completion date is May 15 th , 2024. To help Angel with his issues, the clinician will explore the
relationship between substance/alcohol use and family conflict during individual sessions using motivational interviewing and CBT. The clinician will also help Angel expand his social support network by finding non-using friends. This intervention will take place once a week in a 60-minute individual session for four months. Angel has been struggling with PTSD, which has led to a reliance on alcohol. To address this, the first step is for Angel to attend three Mindfulness-Based Stress Reduction meetings every week. This goal was set on April 12th, 2024, with a target completion date of May 30th, 2024. Additionally, Angel will work on a relapse prevention plan to avoid self-medication, established on the same day with a target completion of May 1st, 2024. Throughout this process, the clinician will provide support and education on alternative coping strategies to help Angel manage their symptoms.
