emphasis on establishing a new and secure connection as the foundation for healing resonates strongly with me, especially considering that relationships are at the core of the challenges trauma clients face. The psychological effects that follow a traumatic experience, such as avoidance, mistrust of others, and difficulties in emotion regulation, can certainly pose potential barriers to forming a strong alliance in therapy (Howard et al., 2022). Other roadblocks hindering a strong alliance may include difficulty expressing emotions, resistance, fear, and therapist competence. While working with trauma clients, the therapist's ability to understand and work with relational trauma is vital. If the therapist lacks experience or knowledge, it can potentially hinder the therapist's effectiveness in building rapport and addressing the client's needs. Additionally, trauma clients may hold fears of vulnerability and trust due to past negative experiences, which makes it a significant challenge to overcome resistance and establish trust with the therapist. External stressors or crises can also impact the client's ability to fully engage in therapy sessions, potentially slowing down progress. Chen et al. (2020) highlight that "trauma-focused psychotherapies may underemphasize rapport building or jeopardize the patient-provider relationship" (p. 8). Therefore, actively addressing and acknowledging these potential roadblocks and fostering a safe and empathetic therapeutic space can gradually contribute to a strong alliance with trauma clients.
