DSM 5 Principal Diagnosis F65.4 Pedophillic Disdisorder-exclusively sexually attracted to males ● An individual who has had arousing fantasies about, urges for, or behaviors with a prepubescent child or children. ● The individual has acted out these sexual desires, or is experiencing significant distress or difficulty as a result of these desires. ● The Individual is 16 years of age, and at least five years older than the child or children noted in Criterion A. (APA,2022) Chris is a 32 year-old Caucasian male. He was the youth director in a large metropolitan church. Chris liked to work closely with boys, he even had an after-school program for boys between the ages of 5-12. He valued the reputation but eventually admitted to having sexual fantasies after serving time for child moilestation. Medical Conditions/ Other Conditions That May Be a Focus of Clinical Att. F65.9- Paraphilic disorder, unspecified;Sexual deviation Need to rule out Non Exclusive type Sexually attracted to both Antisocial personality Disorder F65.89- Other Specified Paraphilic Disorder
EBT Requirement 1: Evidence Based Pharmacological/Medical Treatment(s): ● Naltrexone ○ Reduces impulsive behaviors and therefor may be effective across a wide spectrum of addictions (Richard, 2021) ● Selective serotonin reuptake inhibitors ○ Antidepressant effect ○ Assumed to lead to a general decrease in sexual desire as well as erectile function and delayed ejaculation (Richard, 2021) ● Cyproterone Acetate ○ Synthetic testosterone antagonist ● Medroxyprogesterone Acetate (MPA, Depo-Provera) ○ Synthetic gestagen ● GnRH Agonists (triptorelin and leuprolide) ○ Testosterone-lowering drugs ● Surgical castration (bilateral orchiectomy) ○ an irreversible procedure involving the removal of the testes or testicular parenchyma, leading to a reduction in the level of circulating sex hormones, and subsequent diminution of libido (Winder, 2019) EBT Requirement 2: Evidence Based Psychosocial Treatment: ● Cognitive-behavioral models have proven to successfully treat individuals with pedophilic disorder. ● Individual therapy ● Group therapy
