 I enjoyed reading your post. It sounds like it was a difficult decision to make with “Sarah”. Helping her work through her feelings  and allowing her to decide on alternatives was a great approach. I often experience clients confiding in me about potentially participating in situations that can be dangerous for them, such as refusing to seek Behavioral Health services or no longer wanting to take medications . Although I cannot force them to take the medication if they are not a danger to themselves or others, but it can cause them to have an increase in symptoms or increase in behaviors. Once I analyze the potential dilemma, I am able to determine my next steps. Collaborating with the clients to create alternatives and creating back-up plans in case of an emergency situation is very helpful. The clients that I often encounter that declines treatment explain to me that they find more satisfaction in socialization than therapy and recently I have been working with clients who use natural remedies instead of prescription drugs. According to the NASW (2021) Social workers respect and promote the right of clients to self-determination and assist clients in their efforts to identify and clarify their goals. Social workers may limit clients’ right to self-determination when, in the social workers’ professional judgment, clients’ actions or potential actions pose a serious, foreseeable, and imminent risk to themselves or others (Self-Determination, 1.02).
