The referral information indicates that the client is a 17-year-old Caucasian female seeking individual counseling to manage emotions, primarily influenced by a verbally abusive relationship with a 19-year-old boyfriend. b. Background General Information on Client: The client resides in a single-parent household with her twin sister. Her parents have recently divorced. She has been diagnosed with ADD, Anxiety, and Depression. c. Demographics, Supports: Caucasian, 17-year-old female, residing with a single parent and twin sister. Her biggest support system is her mother. d. Cultural, Social, Spiritual Factors: Exploration of cultural, social, and spiritual factors involves a consideration of the client's Christian faith and its potential impact on coping mechanisms and perceptions of healthy relationships. e. Family History/Relationships: The client's family history indicates a one-parent household and a verbally abusive father, which may contribute to the dynamics in her current relationship. Her mother was adopted and she is unsure of her family history. f. Substance Abuse: No reported substance abuse issues at this stage. g. Social Relationships:
