On 7/23/2015, Client attended to her ILP meeting with her youngest son Marcos. Child was observed well dressed for the weather. Client stated that she didn’t attend to her ILP meeting because her son was sick. Client also stated that she have a boil in her waist. PA/Entitlements/ Benefits: Client has an active PA case. Client had a SSI appointment on 6/2/2015. Pending response Client stated that her husband is attending to the Back to Work program. Cm request client husband to provide proof of attendant. Client states that she has to pay her storage bill. Client stated that her A&A moving storage is located at 1351 Jerome Avenue Bronx, NY 10452. Housing: Client meet with HS and she was explains that she could be able to provide
Housing: Case Manager informed client that she must meet with assigned Housing Specialist on a weekly basis to discuss her housing opportunities.
Entitlement/ Benefits: Case Manager confirmed and verified that Ms. Medina’s PA case is open and active. Ms. Medina is only receiving food stamps. Case Manager encouraged Ms. Medina to visit her PA center for the purpose of obtaining information about her case. Ms. Medina stated that she is still waiting for her Unemployment Benefits to be re-opened. Ms. Medina stated that she is attending to the back to work program.
SOCIAL SUPPORT UPDATE: Client reported she wasn’t feeling well. She has bronchitis and takes prescribed medication. CM inquires if the client got in touch with her children and sister. Client replies “her children live in Florida and she doesn’t know the whereabouts of her sister. She also reported no community support.
SOCIAL SUPPORT: Client continues to complain why DHS didn’t submit her housing package to this shelter. She also states that this worker is mental health and she doesn’t have any mental health disorder to be placed in a SRO. Client also threatened to this worker to call 311, DHS advocate and the Coalition. As she quotes:” is not about you or CAMBA, I am doing this because DHS have all of my information”.
Client reports SSI Fair Hearing was scheduled on 8/6/2015, client mentioned WPA Law Project is assisting the client to hire a SSI lawyer.
On 7/11/2015, CM met with the client for Bi-Weekly ILP Review. Client arrived early for the meeting. In the meeting client appears to be well groomed with good hygiene and dressed appropriately for her age. She was calm, cooperative and well related. She discussed typical issues re: her political and activism work and automobile accident disbursement. She continues to relate her paralegal was in the neighborhood and she wanted this worker to meet her. Unfortunately, there was no parking and she left. She also mentioned the paralegal brought her documents that she need to filed with the state and the federal for her automobile entitlement.
Housing Update: Client housing plan is Project Reconnect and Travel Assistance. Client has no interest to be reunified with her family in
Housing: Ms. Williams is City FEPS certify. Case Manager encouraged Ms. Williams to continue working closely with her assigned Housing Specialist for the purpose of obtain the proper information about housing.
Client came into social service after receiving a SUS appointment slip left at the front desk by CM 10/3/17 to meet on 10/5/17 at 4pm. Client stated she only had a few minutes to talk because she had to cook dinner. Client informed CM her PA account was closed and she replied once again. Client also reports she has to visit Connecticut, PA office for a letter stating her PA case is closed in that state. Client informed her CM all documents requested will be submitted within a weeks’ time. Client reports two of her four daughters Selah McKenzie and Olivia Lue have a doctor appointment tomorrow October, 5, 2017. The other two daughters, Trinity and Nyan McKenzie have appointments next week Tuesday on October 10,
On 7/15/2015, CM conducted a search via Security to locate client. Client was found sitting in the patio. Security asked the client to go upstairs to the 3rd floor to see social services. CM met with the client and completed Bi-Weekly ILP Review. In the meeting the client was well groomed and dressed appropriately for the weather. She appears to be calm, cooperative and friendly. CM inquires how client is doing since the last Bi-Weekly IlP. Client replies she is doing well and she is still waiting for DHS Manifest if she was approved or disapproved for the apartment” CM reiterates to the client as per P4P meeting on 7/9/2015, client was informed to be “open minded” and also there is a possibility she may not be accepted for the housing.
During meeting Ms. Declaire stated that they have not apply for NYCHA. Client also stated that family has never been evicted from any apartment because family has not own their own apartment. As per review and discussing the case with Ms. Declaire they are not eligible for any program at this point due to the lack of income. The only chance that family may have is LINC, client was instructed to provide last four pay stubs to inquire via CMFT for the LINC program. Lastly, family stated that they do not have any family members willing to help them. Family does not want to move out of
On 1/5/2016, she had an appointment with Mr. Jose Diaz/WPA Law Project for SSI interview and assessment for SSI entitlement. Client reported base on Mr. Diaz assessment she doesn’t have a strong case to qualify for SSI and he dropped the case. Client continues to say Mr. Diaz recommended the client to see her PCP and continues to take all medical referral to have a stronger case. Client reported this is the 3rd SSI lawyer that dropped her case.
Client and this CM was able to complete all three forms. Client stated to this CM working on Friday at 12PM to Monday at 12PM each week as home health aide in New Canaan for an 89-year-old women. Client expressed happiness to be employed. Client stated being homeless since June of 2016 when due to getting kicked out of father's household. Client stated needing help to get housed is the reason for requesting case management services. Client stated to this CM being kicked out of father's house was due to drug usages. Client is currently receiving $383 monthly from SNAP and make roughly $1890 monthly from job. Client stated to this CM utilizing the Day Street Clinic in Norwalk CT as a primary care provider. Client also stated to this CM receiving therapy from Rebecca at Day Street Clinic for pass history of trauma. Client stated to this CM being raped three years ago and have a history of sexual abuse. Client stated to this CM medical history contains heart murmur. Client currently is taking 500mg of trazadone nightly. Client is diagnosed with depression and bipolar disorder. Client stated to this CM having 8 children, 6 biological and 2 adopted. Client family history contains depression and high blood pressure. Client scored 8 on the CAGE Assessment and scored 15 on the Modified
Client is undocumented and her housing package is incomplete. Missing the following: Green Card and income.
CM was out on vacation for the period of 5/2/2016 to 5/9/2016. On 5/19/2016, CM met with the client to update Assessment and to complete Other ILP Review. In the meeting client appears to be friendly and cooperative. She appears to have some cognitive impairment. Client reported WECARE/Wellness referred the client to see Dr. Larissa Lempert/Neurology. Next upcoming appointment is scheduled for 5/26/2016. During the meeting session, client was dressed appropriately for the weather and had good hygiene. She ambulates with a cane due to leg problem. Client affect was flat. Client denied suicidal or homicidal ideation.
