Service Coordinator (SC), Jennifer Stoker met face to face with provider, Aiesha Crayton and consumer Jonathan at his group home. Jonathan informed SC he has change his mind he don’t want his own apartment. SC asked why he changed his mind. Jonathan noted Aiesha told him that she couldn’t pay a staff member to live in his apartment with him. He noted Aiesha could be his provided but it will not be 24 hour care for him at his apartment. Aiesha noted she also talk with him about the amount of money he would have to pay for the apartment. Aiesha noted the apartment he want was 700 dollars. An “audit failure” is a situation in which a professional auditor fails to detect a material error in the financial statements of the company they are auditing. The audit failure in the situation of Rita Crundwell the failure was exacerbated by the fact that the auditors continually signed off on the misstated statements for years. Crundwell is responsible for many of the deficiencies mentioned, such as the missing funds and the incorrect invoices. However, she is not the sole person responsible for this fraud. Client (AM) is a 20-year-old heterosexual African American female, born in Durham, and currently still resides there. Her primary language is English. She lives with her 13 moth old child (NM) in a one-bedroom apartment. The client is unemployed and currently receives SSI benefits. AM resides in subsidized housing because of her social security income. Client did not finish high school and has no desire to do so.
On Tuesday July 7, 2015, at approximately 3:01 PM, Kiana Beekman, (MFCU Investigator) (Beekman) received a call on the state office telephone from HILL, Lucy (Service Facilitator of Lucy Hill Services (LHS). During the conversation, Beekman asked HILL to clarify her role and responsibilities as a service facilitator, in addition to the role and responsibilities of HARRIS, LaFrance as the Employer of Records (EOR) for Medicaid Recipient DANIEL, Rose and MCGHEE, Inocencia as DANIEL’s aide. She was also asked to provide any documentation of training on timesheet submission and approvals that she provided HARRIS and MCGHEE under the Department of Medicaid Services (DMAS) Consumer-Directed care aide program.
On Tuesday March 8th, 2016 Case Conference with Resident Ebony Rice #325 has been conducted with Janette Chirico from DHS , Program Director Felicita Rivera, Housing Program Supervisor Zenobia Garland and Senior Case Manager Ms. Arias. The purpose of this meeting was to address client noncompliance and to come with an exit strategy for this Ms. Rice. Client was asked the reason for her missing ILP meetings and her barrier to obtain permanent housing. Client has a LINCH voucher for the amount of $1515. Client stated that she hasn’t receives any help from facility staff so she is searching for apartment on her own. Ms. Rice has been receiving assistance by the onsite housing department but unfortunately she has been able to link to an apartment
At 0824 this clinician met with the patient who reports her weekend was not unpleasant. She reports she is continuing to have problems locating housing. She reports her family members continue to refuse to allow her to live with them specifically her father and her uncle. She reports she has one friend who is considering allotting her the capability to reside with her under the condition that she speak with her Child Protective Services (CPS) caseworker first. She reports she does not want to reside in a homeless shelter with her son as she has done this before and the experience was not up to her standards.
On this date, Matthew Mitchell APS supervisor conducted a meeting on behalf of Mr. and Mrs. Beaird well-being. In the meeting, Mr. Mitchell presented Mr. Joey Beaird and Mrs. Julie Johnson with solutions that would allow Mr. Gail Beaird to return home. However, both Mr. Joey Beaird and Mrs. Johnson declined to take care of their parent independently. Each stated their home was not handicap assessable and they had jobs and could not dedicate that much time to their parents care. The other option discussed was hiring another service besides Visiting Angels since they are refusing to continue services if Mr. Beaird returns home. However, this is the only service offered in Jasper. When the discussion of Mrs. Johnson and Mr. Beaird hiring sitter independently they stated they had tried this before and it never worked out.
Person served was enrolled in PACT services on 7/20/15.While completing his initial 30-day plan, the team developed a list of things he would need to be comfortable and re-integrate into the community. The team assisted person served with communicating with his landlord and advocating for himself, as there was no stove or refrigerator in the apartment. The team assisted him with budgeting and bill payment as a means of preventing any possible conflict with his landlord and housing. The team assisted him with getting necessary food, clothing, personal hygiene supplies, clock radio, etc. Team also wrote a letter to DHMAS requesting additional funds to furnish his apartment, and the team was granted $1,500.00 to assist with his apartment needs. On this date worker received a call from Tori Pettit. Ms. Pettit stated that Serv-Pro had visited Ms. Smith's home did an assessment and sent off the mold samples. The results came back that the house is not livable due to mold infestation. The mold is in the walls, ceiling, basement, and air vents. Ms. Pettis has spoken with Ms. Smith and Ms. Rachel Rodgers about Ms. Smith moving ASAP. Ms. Pettit stated that Tri-County could place Ms. Smith into a group home now due to her home being deemed unlivable. However, Ms. Smith is not interested in-group home placement and would rather live independently. Ms. Rachel Rodgers has agreed to assist Ms. Smith with apartment applications and any other thing need in order for MS. Smith to find residence.

LAMP VASH worker Laurie Garza inquired about client’s drug use and general well-being due to client’s lack of motivation. LAMP VASH worker suggested client explore different housing programs due to client’s resevations regarding weekly meetings with case manage . Over the last two years, Ms. Erica Williams has been a participant in Inspiration Corporation Short Term Subsidy Program. The Program is funded through Housing and Urban Development (HUD), as a way to help persons that had suffered in the grips of homelessness become self- sufficient. Ms. Williams has been a true testament to the outstanding tenacity that many of our participants possess throughout their tenure.
