To identify, my change process in my project plan I will use Kotter’s eight stage change management process. First, the sense of urgency in my project is to keep inspiring my team to make sacrifices to help make the change in guiding my sister in a healthier eating habit. Second, I have selected my parents as team members to help carry out my sister’s change for a healthier diet by developing supporting strategies like printing off words of encouragement for them to use to help keep her motivated. I had no idea that what I included in my diet was so important and provided a significant impact to my wellbeing. After doing the first part of this Analysis project, I was surprised to see the many areas that I can improve on. I created 3 specific, measurable, attainable, realistic, and time-bound goals to help me improve on receiving the right amount and variety of nutrients in my diet. My goals are to drink 8 glasses of water a day, have fruits and vegetables every meal adding to a total of about 4-6 cups, and to have a different breakfast every day. For 30 days, I attempted to implement daily each of my 3 goals.
Week one was designed to perform a preliminary observation about my daily water consumption. This step gave me the baseline data I needed for comparison on how much progress I made at the end of the three-week project. During the final two weeks of the project, I actively tried to increase my water consumption and tracked my progress each day. Therefore, I was also able to make observations about the experiences I encountered. In the paragraphs that follow, I will describe my own personal experiences during the weeks that I actively tried to change my behavior. This description will also include, my initial plan, the strategies I implemented during the project, the factors that played a role in my success or defeat, what the data I collected reveals about my overall change, and the experiences that surprised me while trying to change my daily water
I have learned a lot from this two-day diet analysis assignment. In my two day assessment, I recorded my food intake, quantities, drinks, snacks, condiments, where and who I was with during the meal, the time of day, and the amount of time I spent eating. I was able to analyze my diet to examine what I was eating and what types of nutrients I was getting. I then compared all aspects with my recommended levels of nutrients for my dietary recommended intake. I learned that my eating habits are not the best and could lead to serious health problems in the future that may be detrimental.
The purpose of this assignment is to increase self-awareness of the student by analyzing personal nutritional intake and activity routine throughout the course. In turn, the student will be able to help others modify their diet and activity, and improve overall wellness. An examination of a personal food and activity assessment will provide the student with essential data from which to change their health and wellness behaviors.

1. Eating plenty of fruit and veg- This was good for me to think about when my individual could eat her fruit most important in the day, as before creating the healthy plan she was not eating lot of fruit, it is very important that individuals get their five a day, this could be used when the individual is on the go instead of going to get a take away, she can go and eat some fruit which is going to fill her up as much as the take away but will be better for her health and her body, it will also give her a better nutrition to the body and also her brain for energy. This is also for her veg as well which we need to have every day.
To begin with, the specifics will include encouragement from our parents as well as myself to help her make healthier choices with meals. Further, as a team, we will purchase the different variety of fruits and vegetables so she has alternative options, and we will all partake with her in eating healthier as well. Provided that, a measurable intake will play the main role for this, I researched the average daily intake of servings of fruit and vegetables for her age. Next, I will get my parents to agree upon helping me motivate her in wanting to eat a better balance of foods. For this purpose, of encouraging her in eating good nutrition to better her lifestyle, is indeed a realistic goal to be met. Lastly, the time bound for this project will be for 30 days starting February 1st through March 1st and I will conduct weekly evaluations to keep track of her progress. For the efficiency challenge the change I planned for was monitoring my spending at fast-food restaurants. I realized how much I spent eating out and how unhealthy I was eating. I planned on not spending more than $50 a week eating out.
The goal I chose is to live a healthy lifestyle which in terms will add years to my life. In this transformation I chose to eat healthier, exercise more, drink plenty water and add supplements to my personal wellness plan. I will devote a significant amount of time to each area of this wellness plan over the next eight weeks to get the ultimate results. Below are specific goals and a plan of action that I will be doing over the course of eight weeks; in an effort to make lifestyle changes. I will be reviewing the various area of my health deciding which areas I would like to make improvements. My present situation is that I have gained a significant amount of weight that has made my life a little uncomfortable. I started by making a chart to track my plan of action in each area that I will be working on.
The first step in preparing an action plan for nutrition and diet begins with an analysis of my past records on these things, and of the changes that I made over the course of the semester as I became more aware of these issues. The first study was done during January at the beginning of the semester, and the second study of my nutrition intake was done in at the beginning of March. These two studies are the basis of my nutritional plan, and a similar methodology was used with respect to the exercise plan.
Once a quality improvement plan (QIP) has been developed and evaluation methods determined, a plan for implementing the initiative must be created. An action plan is a series of steps and timelines that will ensure a QIP is implemented successfully (Desjardines, 2011). The models of change that I have chosen to describe are the ADKAR model and Kotter’s 8 step change model. The ADKAR model is mainly used to help identify and drive change as well as a tool to understand any gaps that are needed to strengthen along the change process. It is also a useful framework for planning change within an organization, before implementation, and in the execution phase of the change management process. This process begins with five key goals that are the basis of the model; awareness of the need to change, desire to participate and support the change, knowledge of how to change, ability to implement the change on a daily basis, and reinforcement to keep the change in place. The Prosci ADKAR Model process is best described as change management which focuses on communication and training. In describing the ADKAR process there are five parts to its system that is beneficial any industry making a change to its department or system. In that process of change is the ability to make people aware of the coming change; the next step in the process is to explain the desire for the change; the third step would be to gather understanding and knowledge of the benefits, the fourth step would be provide the group the ability to state their concerns and answers their concerns an needs and finally reinforcement the change
