A reflective analysis on a case presented via a video tape of a simulated patient with a minor injury. This assignment will present a reflective analysis of the examination, diagnosis, treatment and referral plan based on a simulated patient presenting with a minor injury. Potential diagnoses related to the mechanism of injury will discussed aligning the patient presentation with the literature to produce the most likely diagnosis. When this is established, a suggested treatment plan will be created in line with current guidance. This essay is based upon the case of Anne Rutter. A forty six year old lady who two days previously fell on icy path outside of her house; putting her left hand down to break the fall. Anne reports immediately feeling …show more content…
There are many specific types of fracture that occur in the thumb and first metacarpal, fractures of the proximal phalange and distal phalange are simpler to manage than the more complex Bennett and Rolando fractures which involve the joint between metacarpal and carpal bones (Day & Stern, 2010). If a fracture occurs in the first metacarpal, or proximal or distal phalange the patient will experience pain to the thumb, the area may appear bruised and will swell; patient also in most cases experience a loss in function and are unable to complete tasks such as pinching motions (The Royal Berkshire NHS Trust, 2013). Anne is not experiencing any loss of function in her arm and there is no swelling of the thumb; consequently, it is unlikely that Anne has fractured her first metacarpal, distal or radial …show more content…
Anne is currently taking paracetamol for the pain in her wrist, within healthcare analgesia should always be monitored to assess whether it is achieving elimination of pain and should be adapted to the individual patient (Vargas-Schaffer, 2010). If paracetamol were not effective in eliminating Anne’s pain then practitioners should consider a non-steroidal anti-inflammatory medication [NSAIDS] or a mild opioid medication such as codeine as the next step (Vargas-Schaffer, 2010). However as Anne is asthmatic NSAID medication should be used with caution due to the risk of increased frequency of asthma attacks and breathlessness (Joint Formulary Committee, 2015); if Anne has taken NSAIDS before with no issue then this would be the next choice of analgesia followed by codeine if combined paracetamol and NSAID did not prove effective (Vargas-Schaffer,
An elderly female patient presented to the emergency department with obvious deformity in the distal aspect of her forearm following a fall onto her left arm. The patient had fallen backwards onto her left side and landed on her wrist which was in a flexed position. Due to the way the patient had fallen, as well as her age, the primary pathology the doctor had suspected was a Smith’s fracture (also known as a reverse Colles fracture). “Smith fractures are fractures of the distal radius with associated palmar angulation of the distal fracture fragment” (Goel & Gaillard, 2015, para. 1). An x-ray was required to determine if it was a Smith’s fracture or another type of fracture. If the fracture turned out to be
All incidences of injury should be reported, promptly and correctly managed, in a compassionate way. This should be followed by a root cause analysis and an action plan to prevent recurrence which may include retraining staff, and providing necessary equipment among others. Care is offered free of charge to all staff whether paid or unpaid staff as long as they are working at the health facility.
Opioid medications are frequently prescribed for severe pain. Opioids includes the pain medications like oxycodone, hydrocodone, morphine, and fentanyl as well as the illegal drug heroin (American Society of Addiction Medication, 2016). Many people rely on these drugs to relieve their pain from surgery, active cancer, chronic pain and end of life care (WebMD, 2017). Studies from the 2012 National Health Interview show that over 11% of adults report having chronic pain (Dowell, Haegerich & Chou, 2016). The opioid epidemic is a significant issue for nursing on the main respect that there is currently a problem with over- prescribing of narcotics and the millions of people addicted to opioid medications
- Examine the patient for other injuries, such as a neck injury that may have occurred as a result of the backward motion of the neck
Anna Tarlton is my 92-year-old mother and despite her wishes to see me, my sister Nancy Linton is isolating her from me, her other children, my children, and my siblings’ children. My mother, Anna Tarlton, lives with my sister Nancy Linton and Nancy has the Financial and Medical Power of Attorney but uses that abuse of authority as leverage against us and strip my mother for her freedom to visit her children. As a demonstration of this abuse, I have been banned from her property and not even allowed to acknowledge her in public without my sister or a “third party” person listening to our conversation. Moreover, there is no phone, (it is there, but disconnected from the wall and wireless) to call therefore I cannot even talk to her over the
The experiment was conducted on 75 healthy University of Sydney medical science students of both sexes and varying ages and body types. which involved two different types of induction of pain. The independent variable was the type of drug analgesic (paracetamol or combined with codeine) and the dependent variable was the pain response whilst taking the drug. All participants were given ethical and safety awareness of the experiment, and had a choice to participate. Their medical conditions (i.e. allergies) and any previous morning medication taken were considered prior to allowing them to participate.
X-ray of the right hand/wrist dated 09/27/2017 showed an acute fracture that involved the distal metaphysis right radius with 2mm separation. There was a dorsal angulation of the distal segment. There was a subtle avulsion fracture involving the styloid processes of the distal ulna.
Displaced fractures, right hand. By radiographs the middle or long finger metacarpal base and the ring metacarpal base and equivocally from records and current x-ray, possible small finger metacarpal base injury.
After medicating Patient Y, the nurse was able to determine that the medication did work based on the patient’s pain score. Patient Y mentioned to the nurse that she did not like wearing the abdominal binder, but holding the folded blanket on her abdomen while coughing was very helpful. The nurse’s interventions were effective. By the end of the nurse’s shift, the patient was rating her pain a 5 out of 10 consistently.
It may be to your benefit to have us examine your case as the circumstances and the severity of your injury will determine the
Paracetamol, also known as acetaminophen belongs to a class of drugs called analgesics (pain relievers) and antipyretics (fever reducers). The exact mechanism of action of acetaminophen is not known. Acetaminophen relieves pain by elevating the pain threshold, that is, by requiring a greater amount of pain to develop before it is felt by a person. It reduces fever through its action on the heat regulating centre of the brain. Specifically, it tells the centre to lower the body’s temperature when the temperature is elevated. Acetaminophen relieves pain in mild arthritis but has no effect on the underlying inflammation, redness, and swelling of the joint.

Whenever discomforted by experiencing pain, one ingests certain analgesics to relieve and alleviate the pains throughout their body. A common misconception is that analgesics can be used to cure the pains and diseases within one’s body. (Roberts, 2006). Analgesics can also be referred to as painkillers, as they are used to relieve pain. (“Analgesics”, 2015). Like the majority of other drugs, analgesics become poisonous after their expiration date and should not be used in children younger than the age of 2. (Roberts, 2006). Analgesics can be purchased at pharmacies and specific grocery stores. Generally, there are three types of analgesics stored in pharmacies: simple non-opioid analgesics, compound analgesics, and opioid analgesics. (Shipley
Since the worker suffered such a traumatic injury, it is imperative that the employer first check on the employee to check on the severity of the injury and upon assessment by a trained employee, either the correct medical procedure needs to be administered to the employee or they should call the ambulance so that the employee can be safely transported to the hospital, preventing further damage and ensuring that all measures are in place to repair the injury.
DOI: 04/04/2012. This is a 42-year-old male right-hand dominant warehouseman who sustained a work-related crush injury to his right hand due to working on slitter, causing amputation of the right thumb. He is diagnosed with right thumb crush injury. Per initial evaluation dated 03/09/2016, patient is in a temporary thumb spica splint. Physical examination reveals post-operative wound at the right dorsal/radial forearm, palm, and ulnar wrist. There is minimal swelling. He has no current functional use of the right thumb. He is unable to write and pick up small 1cm-size objects with accuracy. Right thumb metacarpophalangeal joint extension/flexion is 30/35 and right thumb interphalangeal joint extension/flexion is 45. As per 6/7/16, the patient
