Elder Assessment Instrument (EAI) Findings The general assessment findings regarding the patient ranked the general assessment as very poor regarding clothing, hygiene, nutrition and/or dehydration, and poor findings regarding skin integrity. The general assessment showed that the patients clothing were not clean; saturated with urine and food stains. In regards to nutrition this ranked very poor as well, considering the patient has lost 10 pounds since her last hospitalization only two weeks ago. This weight loss also could be because of dehydration or nutritional imbalance; further labs will need to be completed to decide if weight loss was regarding nutrition, dehydration or both. Albumin levels will be assessed for nutritional status (Bharadwaj, 2016). Skin integrity ranked a poor on the scale. The diffused red abrasions noted in sacral area could be a stage 1 ulcer. (Advanced Tissue, April 2014). Being unkempt, soiled and dirty is large warning sign of neglect especially for elderly patients (HelpingGuide.org, 2017).

While enroute I learned Trooper Dingman was out a 30 Marie Ave investigating. I arrived and found Trooper Dingman speaking with a male & female inside of apt. #1. Trooper Dingman briefed me there was an altercation between Linda Showalter, Jessica Goose, Eli Raisch, & Valerie Hartmann. Trooper Dingman stated there was a call for service about 46 hrs. ago where Valerie reported a family member of Jessica’s was inside Valerie’s cleaning when she was accused of stealing pain medication. Valerie & Linda then confronted Jessica & Eli about it, and an argument ensured.
While during round today on Thomas Unit Justine P disclosed that she had been collecting sharps all day. Justine were able to pull several pieces of plastic ware (Spoon/ Fork), a stitch from her injured wound, from her bra also and a rubber band. Campus Supervisor was able to retrieve all items from her. Also Justine disclosed that she removed all the stitches from her wound and began to embed new items into her wound. Nursing was contacted for assessment.
The case was brought to DCFS attention when a reporter stated on 7/11/17, OPWI attempted to obtain an emergency order of protection on behalf of Kiara (age 7). Reportedly Ashley (mother) is actively using METH and also methodone she gets from a clinic, and is also involved in prostitution. Ashley told OPWI that Ashley ''can't wait outside for the bus to go to drug court because she's done bad things to people (robbed people, committed home invasion, taken people's money and drugs) and these people are after her.'' Reporter states that one of these people came to OPWI's house looking for Ashley and scared Kiara. Ashley recently got out of jail, stayed with OPWI for a couple of days, left last Friday telling Kiara she would be back in a couple of hours, didn't return
During the time of assessment the patient was watching television with his mother on his bedside. The patient report that another student gave him a razor off the floor and he wanted to make marks on him that looked like a cut that the patient reports appeared in his sleep. The patient appears guarded with a flat affect. The patient starts to talk in a low tone and only respond by head movements.
The reporting party (RP) stated resident Rita Henderson was brought into the hospital due to gangrene of her right heel. According to the RP the resident was treated in Eisenhower Medical Center located at 39000 Bob Hope Dr., Rancho Mirage 92270 (760) 346-7772 in February 2017 and release on 2/26/17
I, Cpl. Lessane, along with Deputy Ayer, responded to Hampton Regional Medical Center, to assist Deputy Smalls, with a runaway mental patient. Upon arrival, Deputy Smalls stated a mental patient, Travis Johnson, ran away from the Emergency Room with a set of black handcuffs on one of his arms. Deputy Smalls stated he wasn’t sure of Travis exact location at the time. Deputies along with assistance from the Varnville Police Department and the Hampton Police Department patrolled the area in an attempt to make contact with the patient. No contact was made at that time.
The victim was transported to the hospital and was diagnosed with a broken nose, fractured rib and did not require stitches for the laceration to the side of his head. The victim met with Detective Cornatzer on 02/05/2016 at 311 County Street in reference to this case. The victim provided the names of Donavan Sneed and Jerry Harding as two of the suspects. Shockley’s oral report, Mr. Wood related the occurrence to Dee Showalter, our client Human Resources Director, who in turn reported the matter to Mary Miller, our client’s Chief of Living and Learning. Ms. Showalter also contacted our client’s nurse, Janice Young, and requested that she perform a skin assessment on Allison and also arrange for a physician to examine her per Ms. Miller’s request. (We have requested a copy of Ms. Young’s skin assessment, though the portion of the file that our client provided does not include a skin assessment that was ordered at that time and our client has indicated that it does not have a record of a skin assessment performed on or shortly after September 9, 2013. Noah Scott was prompted to the shower to complete activities of daily living. Noah Scott complied with the demand and walked to the bathroom. While in the bathroom, staff Watson Suah prompted Noah Scott to take his clothes off and Noah Scott complied. While Noah Scott was taking his clothes off, Watson Suah discovered the following injuries on Noah Scott: bruises on upper right and left arm, and left and right rib cage.
Her throat had been severed, and then her body had apparently been moved to the middle of the bed and eviscerated. Much of the skin had been removed from her thighs, and her breasts had been cut off by circular incisions. One of her breasts was near her right foot and the other was under her head. Her intestines were by her left side, her liver was between her feet, and on the table next to the bed were large flaps of skin and muscle that had been removed from her abdomen and her thighs. Matthews LR, Danner OK, Ahmed YA, Dennis-Griggs DM, Her physical exam was notable for a blood pressure of 105/56 mmHg, pulse of 87 beats/min, temperature of 37.7 C, respiratory rate of 20/min, and oxygen saturation of 100% on the ventilator. Her secondary survey revealed unequal pupils with discordant reactivity. Her right pupil was 8 mm and non-reactive to light and her left pupil was 3 mm and reactive to light. Ominously, she was noted to have decerebrate posturing (indicating severe brain damage) of both the upper and lower extremities bilaterally. On further examination, a five cm laceration to the right lower anterior thigh was identified and repaired. Her Focused Assessment Sonogram for Trauma (FAST) exam was negative.
Respiratory Examination I then needed to carry out a respiratory assessment. I observed Mr Brown’s chest for any visible signs of scars or trauma. This appeared normal.
