Assessment Question In order to provide the best assessment for Diane, we as a team need to focus on our assessment question as follows: Who is Diane as a person, and what are her unmet needs in her daily life, especially in regards to her current mode of communication and her interactions with family/partners; additionally, are there any barriers currently halting progress? It is vital that Diane’s best interests are always on the foreground; her own priorities and her own desires will lead our assessment into future interventions. We are able to take apart the above question in four steps in order ascertain why this is a valid goal to attain for Diane.
First, it is important to assess Diane’s overall unmet needs. In understanding one’s unmet needs, we are able to assist Diane in engaging in her own interactions and activities in her daily life (Beukelman & Mirenda, 2013, p. 8). Research has shown that children and young adults with cerebral palsy are at risk for having a high amount of unmet needs, especially in relation to health care (Jackson, Krishnaswami, & McPheeters, 2011; Nieuwenhuijse et al., 2007). As health care professionals, we need to be aware of Diane’s unmet needs in order to identify her own personal goals and skills in order to plan an accurate model of intervention.

I attended the Centennial Christian School to visit one of the student, Ashley, who is diagnosed with cerebral palsy. It was interesting to see Ashley’s care plan that was written by Chris Robberts that outlines care of her stoma, flushing of water every hour at school via feeding tube, administering mediation via feeding, and what to do when the student has a ‘blank stare’ or grand mal seizure.

Based on the assessment, I am characterized as an Introversion Sensing Thinking and Judging (ISTJ’s). My mental dominate function is Sensing introvert (Si) which means that I value past experiences. I work very hard to fulfill commitments by asking questions and setting appropriate time to work on it. When I am in any group setting, I personally prefer clear team roles before working on an assignments because I do not have to take on extra tasks. I make majority of my decisions based on my feelings and my past experiences. For instance, how I dress is based off my emotions. I tend to be very quiet and serious. I am very supportive of my families and friends decisions in life. Often, I assist my friends with an assignment that they don’t have a clear understanding of such as statistics or nonprofit organizations. I am sympathetic and understanding of people situations. We all at one time been in a rough spot. I take my responsibilities serious. For example, I tend to be very hard on myself, if I score low on an exam.
The multi-dimensional assessment involves gathering, analyzing and synthesizing the client’s situation while focusing on strengths- to develop a plan that involves all the relevant parties and levels (Hepworth, Rooney, Rooney and Strom-Gottfried, 2013). To completed this assessment I would include detail information about Elaine’s family, medical history, available resources, social history, sexual abuse, medical, education, employment, recreation and legal issues. To complete Elaine’s Multi- Dimensional assessment, I would need her family members, social security numbers, date of birth, phone number and addresses. In additional, the worker would need to observe Elaine in person. The worker also need additional information regarding Elaine’ side of the family along with her husband side of his family. My culture, interaction with family and friends, and the various experiences from co-curricular and extra-curricular activities have enabled me to develop a multifaceted personality to tap my potential. In particular different activities, projects, assessment reports, and feedback from colleagues over the last few months at Babson I have better understood my strengths and weaknesses, and work style and leadership style. In this paper, I have developed my self-assessment analysis based upon qualitative and quantitative materials I have, and my experiences and exercises during this semester. I have identified three broad themes that incorporate several subthemes representing my current abilities, preferences, patterns of strengths, etc. (see appendix 1). Beyond the unique therapy that would be required to address her impairments, the communication I observed is what I found truly remarkable.
