The reporting party (RP) stated there is a belief that the licensee and Maria Dickerson are committing fraud. According to the RP the licensee is never available. The RP stated Maria Dickerson lives in the licensed home with her son. The RP stated when they arrive to the facility Maria is the person providing care within 10-15 minutes the licensee would arrive at the home. There were a few occasions when the RP made visits she only communicated with the licensee over the telephone. The RP stated the licensee never answers the phone call made to (916) 806-8085 however, the licensee would return calls made to that phone number.
The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.
The reporting party (RP) stated that 92 year old resident Delora Lovelock DOB: 1/15/24 was residing in the facility from 5/15/2015 to 6/4/2015. Prior to admission the facility was informed that the resident had a broken femur and could not bear weight. The resident required two caregivers to transfer. Initially the facility assessed the resident and accepted her into the facility. Subsequently the resident was not provided the care necessary. The resident was transferred by one caregiver that resulted in the resident's trip to the emergency room where she was diagnosed with contusions to her ribs. The RP stated that prior to admission that the caregivers were trained in transferring resident however the caregiver had no idea how to transfer a resident from bed to
CW went to 1261 Everton Dr, Apt 9 Akron, OH 44307 because they were called out by Akron Police Department (APD) for allegations of abuse towards 2 children. CW Jonathon Henry and CW Sonya Cole pulled up to the apartment complex and observed that there were over 15 people sitting outside. Both CW could tell that this was the correct address because there were 3 police cars that were sitting outside. As the CW's were walking towards the apartment complex an APD officer asked if the two workers were from CSB. CW Cold responded that they were. The officer then told CW's that there were not 2 children, however, there was 3. The officer stated that there was one child in the bedroom sleeping that they did not know was in the home. CW's were also told by the officer that Mr. Brian Burrell (AP) was in the home and that he was very agitated. The officer told the CW that they had called him down, however, suspected that he would get upset after entering the home. The officer also said that he already knew that the kids were in the custody of CSB and that he would most likely not be happy that the two CW's were at the home. The officer the led both the CW upstairs to apartment 9.
On 3/13/16 SO EMT Perez was notified that an engine had come on campus. SO EMT Cunningham responded along with SO EMT Perez and caught up with the engine by lakeside commons. The driver of the engine stated that he was responding for a lift assist to 3144/3146 apt 220 but that he wasn’t sure which one he was going too. SO EMT Cunningham and SO EMT Perez decided to respond to FR-220 first being that they were the engine came from gate 2 and they were closest to that building. SO EMT Perez and SO EMT Cunningham knocked and announce their presence at FR-220’s door and were both verbally invited in. The resident a Mrs. Helen Neff stated that she could not get out of her chair because it was stuck and needed help getting out of it. SO EMT Perez
Reporting party (RP) called CCIB to cross report that resident Gerald Gilliland (DOB: 4/22/41) reported that while he resided at this facility, unknown male staff locked him in a room against his will. It is unclear what room the resident was locked in because resident was unable to convey any details about the room. Resident then stated that it was all of the nursing staff who locked him in the room. Resident was hospitalized at Kaiser South Sacramento on 9/18/16 through 9/22/16 and is currently residing at Eskaton Greenhaven. RP stated that it is unclear if the resident has Alzheimer's or not. RP stated that the original reporting party is Sacramento Sheriff's Department (Report#16-286176) and RP will forward the SOC 341 to CCIB.
At 9:55pm RA on call Mignona Hall received a call from Public Safety stating that resident Erin Hynd was distraught in her room due to excessive flooding. When RA Mignona Hall was allowed to enter the room, she immediately heard water running in the bathroom. The flooding in room 114 A was excessive and spreading rapidly. Erin Hynd reported to RA Mignona Hall that she had already spoke to Public Safety, Maintenance On Call, and the RHD on call, so RA Mignona Hall contacted Berry RHD Sarah Price. RHD Sarah Price offered Erin Hynd alternative housing with her RA, which she declined stating she had secured housing with RA Ashley Thompson who is also her friend. RHD Sarah Price, RA Mignona Hall, and Erin Hynd all placed extra towels on the floor,
The reporting party (RP) stated her brother, resident Luis Gallegos DOB: 8/26/70 was transferred to the facility from a skilled nursing facility (Premier Care). According to the RP the resident was admitted to Premier Care on July 13, 2016. The RP stated prior to her brother's stay in Premier he was able to make his own decisions and therefore didn't require a POA or a conservator. Consequently, Premier transferred her brother without notifying his authorized representative, his sisters. When the RP and her sister attempted to visit the resident they were refused. A caregiver named Jessie informed the RP that she had no authority to remove the resident. The RP stated her sister Chelsea visited the resident was not allowed to visit in private.
REPORTER: The reporter/ daughter (Kathy) called to report abuse for the victim, Susan. Susan’s vulnerability is her age, and she needs assistance with her ADL’s. The reporter said Charles Mims is abusing the victim. On yesterday (04/27/2016), Susan called the reporter and she was crying. The reporter said she could hear Charles cursing and hitting the victim on the leg. The reporter said Charles forced Susan to sign over the deeds to her home, and he (Charles) owns the home. Charles will not allow anyone on the property. The reporter and her sister would like to care for the victim, but they are afraid of Charles. Susan was receiving services through Home Health, but the services were ended. The reporter said they are trying to get an extension on the Home Heath Services for the victim.
The reporting party (RP) stated resident Sofia Miramontes age 27 DOB: 11/23/88 has developmental delays, deaf in right ear and is bipolar. The RP stated the resident was issued a government cell phone, however the caregivers take it away from her and refuses to let her charge it. On 6/13/16 the police were called to the facility due to resident's inability to enter the facility. The North Hollywood Police Officers Winter (#38878) and Officer Radke (#39569) arrived to the home. The Officers removed the resident due to her being battered and concern for her safety. The Officer informed the Director of agency Sherry Winston that the resident was coherent and walking to her boyfriend’s home. The RP contacted the facility and spoke with a staff
On January 16, 2016, less than two months into her tenancy, Plaintiff threw a party with a neighboring tenant. Several tenants called Ms. Miner in the middle of the night to complain about the party and the aggressively loud noise levels coming from the halls, resulting in police involvement in efforts towards shutting down (Exhibit 5). Moreover, Plaintiff received a personal email from Ms. Miner two days later, notifying her of the complaints filed and to clarify that she was residing in a peaceful residential community where the tenants do not throw or want to hear loud parties. Instead of taking accountability, Plaintiff blamed her neighbor for the disturbance in addition to minimizing her involvement.
Reporting party resident Michale Dean Lopez (DOB: 11/3/58) stated that approximately 2 months ago, that he was lying down in his bed and felt something behind him. Resident stated that he jumped up and turned around and resident Chris (last name unknown) was standing at his bed side with his boxers down and his "thing" sticking out. Resident noticed that his boxer were down half way. Resident does not know for sure if Chris penetrated him but stated that he felt funny and that he takes Ambien along with Trazodone. Resident reported this incident to staff Zack who told staff Nancy. Resident wanted to contact law enforcement but Nancy told resident not to worry and that everything would be okay and to speak with Mr. Beasley. RP stated that he
After talking with Ms. Tucker worker visited Mr. Ashby. When worker arrived Mr. Ashby was sitting on the front porch talking with his neighbors Ms. Posey and the other lady. He was dirty with what appeared to be spit from chewing tobacco and he smelt like urine. Worker asked Mr. Ashby if he wanted to talk privately inside. He stated we could talk in front of the neighbors. Worker asked about the housing authority visit to the home
The reporting party (RP) stated that resident Loretta Brown was served a 30 Day Eviction Notice dated March 1, 2016. The resident is required to vacate the premises by April 1, 2016. The RP stated the notice does not appear in compliance with Title 22 requirements. The RP requested the administrator Dolores Davis to rewrite the eviction notice with the correct language/language was missing. The administrator refuses to rewrite the noticed and referred the issue to the Licensing Program Analyst (LPA) assigned to the facility Mary Trinh. The LPA agreed with the administrator stated that the notice is in compliance with the regulation. The RP requested to have the language corrected on the notice and to have a new issue
The reporting party (RP) stated that former administrator/owner Edwin Ingan is driving resident#1 car. The resident's former neighbor inquired about purchasing the vehicle and resident #1 refused. A while later Edwin cajoled resident #1 into signing Department of Motor Vehicle paperwork. The resident has mild cognitive impairment and memory loss. Edwin has been having resident #1 to sign checks to pay the facility fee. According to the RP each month Edwin would come into the facility and bring a check for resident #1 to sign. Approximately 1 year ago Edwin asked the caregiver to check resident #1 belongings for additional checks for the resident checking account. Fortunately no additional checks were located. The former administrator directed
