At this location, we were able to make contact with the victim’s mother, Jodie Williams, to begin our investigation. Jodie Williams advised this morning at 0630 hrs; she dropped her son Philip Barrett, Jr at her sister's house to be taken care of while she goes to work. Around 0739 hrs, her sister Wendy Williams, sent Jodie a text message asking why does Jr have bruises and scratches on his face. Jodie Williams advised at this time she responded, her oldest daughter Sophia hit him with a toy while they were playing, but asked for more photos. When Wendy sent her pictures of Barrett Jr, she immediately left work and pick up her son and took him to the hospital. Jodie Williams advised a doctor stated the bruises might have been caused by a hand or a dog.

I observed bruises on the left side of his face, ear, and above his right eye. The bruises looked fresh, and they were purplish and reddish. I also observed several scratches going down across the left side of his face. I took four photographs of Barrett’s injuries and placed them into PCSO Property and Evidence.
I asked Jodie if her son had these bruises on his face when he arrived at her sister's house and she advised he did not and her boyfriend Dylan Sims will say the same thing. Jodie Williams advised she thinks her sister dog may have caused the bruising and scratches.

Your affiant spoke with Hope BAKER and Ernesto TAPIA who related that Leslie TAPIA was drinking alcohol throughout the day. Leslie TAPIA assaulted Ernesto TAPIA (her father) and Hope BAKER during an argument. Both Ernesto TAPIA and Hope BAKER sustained injuries caused from Leslie TAPIA scratching them.
Per Reporter: Donna (James lawyer) contacted CPS 9/18/17 with concerns of physical abuse towards Preston. According to Donna, James (victim’s father) went to pick up Preston 9/16/17. During that time Denise told James that she hit Preston so hard in his face it made her hand hurt. There were no marks or bruising noticed. It is unknown of when the incident happened or what caused the incident. James was recently court ordered to receive unsupervised visitations. Kayla (victim’s mother) has supervised visitation with Preston. The allegations were not discussed with Denise.
REPORTER: The reporter (Gennette) called with concerns for the victims, Leslie, Tapenga, and Teagan. According to the reporter, there was a police report in Leslie’s file from Wisconsin. When Justin Tegelman (Leslie’s brother/special needs/currently 19 years old) was 16 years old, he had six counts of sexual assault (from December 2011-June 2012), while living in Wisconsin. The incidents occurred when Justin was a minor. In the police report, Chad (Justin’s father) caught him doing something to one of his sisters (Tapenga). While on the bus the week of 08/17/15, Mattie bumped her head while sitting in her sister’s lap. The child had a gash in her head that bled all over the bus. Mattie may have needed stitches but she was not taken for medical treatment. The bus driver stopped the bleeding and bandaged the child’s wounds; the driver also told Mr. Cleveland what had happened and suggested she be taken to see a doctor. It was rumored that Mr. Cleveland did not take Mattie for treatment and made statements that she would be okay. R/s Kavon asked to ride with the cousin to the store, but he was told no. R/s reportedly Kavon jumped on the running board of the moving vehicle. IRTC spoke with Detective Kenefick who reported that this case was transferred to him because of his recent involvement with this matter. Detective Kenefick conveyed that he initially received this case in February of 2017 with concerns pertaining to two separate children that the alleged subject reportedly sexually abused. Detective Kenefick voiced a few weeks ago, similar concerns were received with concerns pertaining to sexual abuse regarding the subject child against the alleged subject. Detective Kenefick indicated that the subject child and several other children were interviewed at the Manhattan CAC and there were wasn’t enough evidence for a criminal arrest. The reporter said she is concerned and worried about the child. REPORTER: The reporter/not related (Amy) called with concerns for the victim, Dayne. According to the reporter, Mindy (mother) doesn’t work, and she wears a neck brace. Mindy has dizzy spells, but she continues to drive with the child in the vehicle. The reporter said Mindy is not caring for the child. Mindy claims that she is sick to get rid of her child, and Gary keeps the child every night but he is not abusing or neglecting the child. IRTC spoke with the source from Case ID 26668668 who reported that all the children except for Gianna (reportedly at a friend’s home) are at the hospital, but staff members are unwilling to discharge the children until NYCACS is present. Source confirmed the narrative of the report and expressed that the subject child was reportedly engaged in a physical altercation with another child in the home. Yes, Tavion’s mother’s statement of concern about the suspicion of abuse should warrant a valid reason to lunch investigation in the child injury. Moreover, the emergency department staff had suspicion about the Tavion’s injury due to the fact injury is not consistence with an injury sustained in park playground. Hospitals are mandate by law to report any suspicion of child abuse; the Child Abuse Prevention and Treatment Act enacted in 1974 was designed to encourage the reporting of child abuse cases, and prevention. In addition, most states have enacted laws to further protect abuse children, and most states protect the individual required to report cases of suspected child abuse; the following individual are eligible to report suspected cases of abuse include healthcare administrators, physicians, interns, registered nurse, chiropractors, social service workers, psychologists, dentists, osteopaths, optometrists, podiatrists, mental health professionals, and volunteer in healthcare facilities (Pozgar, 2014).
On Oct 8, 2012, a counsellor from Terry Fox Secondary School brought forward concerns of Sally Brown being abused. Mr. Highcliffe reported that he noticed some bruises on Ms. Brown's upper arm and shoulder. When Mr. Highcliffe asked Ms. Brown about these bruises, she replied with a vague story of being involved in a game with her friends and collided with a wall. To investigate, I visited Ms. Brown at her residence and was informed that her parents were away for an overnight prayer vigil at their church. From initial appearances, Ms. Brown presented as depressed and insisted that I leave. I was able to establish a rapport with Ms. Brown and discussed my concerns about her noticeable bruising. Upon agreement, I took Ms. Brown to the emergency ward of the Prince George Hospital where Dr. Rose, examined her arm and shoulder. From Dr. Rose's medical opinion, the bruising did not appear to be consistent with her story of running into a flat surface such as a wall, rather appeared to have been caused by a blow by a heavy instrument. I confirmed with Dr. Rose that he would be willing to testify in court to his findings about the bruises. On return, Ms. Brown told me that she had not been abused by her parents and that she felt safe at home. From further
