During this reopening meeting, the client spoke about her volunteering, her desire to be independent, and motivated for employment. The IPE was written by Pat Gilliard and based on the client’s individual needs all agreed to thirty days in JRT, job coach services, and the vocational goal of transporter. She entered JRT to build work skills, work with the job coach, and participate in JPI classes. The Basic Life Support class with Trident Technical College was rescheduled due to a miscommunication on the date/time. The client was allowed to stay in the training center until the class could be taken. While she completed the course, the client was not able to pass the test after taking the class; therefore, she did not receive the BLS certification needed for transporter positions. Ms. Sieling reached out to the Continuing Education Department at Trident Technical College and they agreed to allow the client the opportunity to sit in the class again and retest. During the visit Ms. Stoker dry heaved into a garbage bucket, and visited the restroom once. The worker was at the residence for about 2 hours. Tanya reported that they received a monthly maintenance service order for ERS but did not get one for the installation and requested that SC put it in ASAP. Kathy has a weight bearing restriction in place, is currently utilizing a wheelchair for mobility, and is a hands on one person assist with transfers. Judy shares that the previous day Kathy and the physical therapist worked with Joe and herself on transfers into and from the car so they can take her to her scheduled appointment with the orthopedic doctor on Friday. Judy shares that it is expected that the doctor will modify the weight restriction at the appointment. Judy and SSA spoke about a time frame for discharge. Judy shares there is no time frame established. Per Judy, Kathy’s private insurance only has twenty days of physical therapy remaining in the plan and does not know what happens at the end of the twenty days if Kathy needs additional therapy. She has plans to contact Humana later this day to discuss Kathy’s insurance benefits. On this date, Matthew Mitchell APS supervisor conducted a meeting on behalf of Mr. and Mrs. Beaird well-being. In the meeting, Mr. Mitchell presented Mr. Joey Beaird and Mrs. Julie Johnson with solutions that would allow Mr. Gail Beaird to return home. However, both Mr. Joey Beaird and Mrs. Johnson declined to take care of their parent independently. Each stated their home was not handicap assessable and they had jobs and could not dedicate that much time to their parents care. The other option discussed was hiring another service besides Visiting Angels since they are refusing to continue services if Mr. Beaird returns home. However, this is the only service offered in Jasper. When the discussion of Mrs. Johnson and Mr. Beaird hiring sitter independently they stated they had tried this before and it never worked out.
On Tuesday July 7, 2015, at approximately 3:01 PM, Kiana Beekman, (MFCU Investigator) (Beekman) received a call on the state office telephone from HILL, Lucy (Service Facilitator of Lucy Hill Services (LHS). During the conversation, Beekman asked HILL to clarify her role and responsibilities as a service facilitator, in addition to the role and responsibilities of HARRIS, LaFrance as the Employer of Records (EOR) for Medicaid Recipient DANIEL, Rose and MCGHEE, Inocencia as DANIEL’s aide. She was also asked to provide any documentation of training on timesheet submission and approvals that she provided HARRIS and MCGHEE under the Department of Medicaid Services (DMAS) Consumer-Directed care aide program.
On 7/11/2015, CM met with the client for Bi-Weekly ILP Review. Client arrived early for the meeting. In the meeting client appears to be well groomed with good hygiene and dressed appropriately for her age. She was calm, cooperative and well related. She discussed typical issues re: her political and activism work and automobile accident disbursement. She continues to relate her paralegal was in the neighborhood and she wanted this worker to meet her. Unfortunately, there was no parking and she left. She also mentioned the paralegal brought her documents that she need to filed with the state and the federal for her automobile entitlement.
On 8/7/2015, CM met with the client and completed Bi-Weekly ILP Review. Client arrived early for the meeting; she was alert, satisfactorily groomed, and casually dressed. In the meeting she was cooperative and friendly. CM inquires how client is doing since the last Bi-Weekly ILP Review. Client reported she is exhausted from running around obtaining relevant documents to be submitted to Lemle & Wolfe, Inc. by 8/3/2015. The doctors were pretty sure that Pierre had less than 6 months to live, so he fell well within the guidelines for hospice coverage. Should hospice coverage be limited to those with 6 months or less to live, or should other be covered? 6 months is a random figure and I think that the covering other could help families deal with hospice care a little better; it would give them a better understanding of what hospice-type services is entailed. It also could be too expensive for covering others and how can you know for sure if 6 months is suitable.

This month we want to recognize our two volunteers Connie and Paul Steber who have been volunteers for Hospice since June and considered it a true blessing to share the stories and experiences of patients who are living through a very emotional and difficult time. They are honored to be making a difference and to be able to share the stories of the patients like James Ellis best known as the Coach, who enjoys their company but most of all watching a football game, especially if his favorite team is on the winning end.
