On this date worker visited Walker Rehabilitation Center in Carbon Hill, for the purpose of monitoring Ms. Blankenship's placement. Worker spoke with Ms. Ana Dyer and learned the family was getting assistance with housing and transportation from a local church. Ms. LaVone Yarbrough and her husband run a ministry that helps family reunite. However, Ms. Dyer is concerned about discharging Ms. Blankenship to the home with narcotic and according to Ms. Dyer, Crimson's boyfriend lives with them. Worker has not been able to conform. Ms. Dyer stated when Ms. Blankenship discharged from the NH she would need a doctor and transportation to the doctor appointments. The nursing home would only discharge Ms. Blankenship with enough for medicine for a few
R/s Alene Collins suffers from arthritis, depression, hypertension, a history of fracture arm, a history of right hip fracture, and she is hard of hearing. R/s Ms. Collins lives alone and needs assistance with some of her ADLs, she has 10 hours of care from CLTC. R/s Ms. Collins has a history of falling so she has a lifeline. R/s Ms. Collins is having difficulty breathing due to mold and mildew in the home. R/s the roof has a leak and according to Shala Martin (daughter), they don’t have insurance to fix it.
On this 9/3/15 worker visited the residence of Mr. Alvin Higgins, for the purpose of making first victim contact. Mr. Greg Nunn, RN with New Beacon Hospice greeted worker outside of the residence. Mr. Nunn stated he called the residence about 3 1/2 hours before arriving and Ms. Betty Griffith was not home. However, her daughter Gina Craven and her boyfriend, Kenneth Woods was at the home. When he arrived, the door was unlocked and Mr. Higgins was lying in his bed located in the living room. After being there for several minutes, Mr. Woods came into the living room and asked if he needed anything. Mr. Nunn told him no and Mr. Woods walked toward the back of the house. Medically Mr. Higgins diagnoses are late stage Alzheimer's. He is bedbound, hard of hearing, and unable to communicate.
On this date worker visited Ms. Joy Lavender at her home 599 Ponderosa Circle, Sylacauga Al. 35151, for the purpose of gathering information on Ms. Lavender's financial history. The information needed to complete the Medicaid application. When worker arrived, Ms. Lavender greeted worker at the door but pretended to be someone else. After talking with her several minutes Ms. Joy Lavender stated she was in fact Ms. Joy Lavender and her mother was Ms. Wilma Lavender. According to Ms. Joy Lavender, she lied because she doesn't want anything to do with her mother. Ms. Joy Lavender stated that several months ago she moved her mother into her home. However, her mother only caused problems and decided she wanted to return to her home in Pleasant Grove.
On this date worker visited Shadescrest NH, for the purpose of case monitoring. When worker arrived Mr. Hopson was lying in bed sleeping. He was appropriately dressed with good personal hygiene. During visit worker was unable to wake Mr. Hopson. Worker spoke with charge nurse and learned Mr. Hopson was doing well. Mr. Hopson became long-term Medicaid approved in Sept. beginning in August.
At Care Springs, SSA visited with Kathy. Also present was Judy Baldwin and Kathy’s Aunt Jane. Jane is also a patient of Care Springs and is Kathy’s assigned roommate. We discussed that Kathy is doing well. Kathy shares details of her fall which resulted in her injury; sharing while walking up the ramp to her apartment she made a sudden stop and turned to greet the Maintenance Man for the complex, lost her footing because of the sudden stop and fell. She shares that the pain level of the injury is for the most part tolerable, while Judy shares that Kathy is currently on a routine dosage of pain medication for the injury. Judy explains the pain medication has been prescribed routinely because Kathy wasn’t requesting the medication when it was

When worker arrived Ms. Phillips was in bed sleeping but woke when her name was called out. Ms. Phillips was dressed appropriately with good personal hygiene. Ms. Phillips did not remember worker from previous visits but was friendly with worker. During visit, Ms. Phillips talked about her roommates and appears to have adjusted well to her new living arrangement. This was a concern for Ms. Phillips because she went from a room for two into a room of four. Ms. Phillips stated she enjoys the food but doesn't participate in activities. Ms. Phillips stated the only time she leaves her room is to eat and get a bath. Worker

When worker arrived, Ms. Salter was lying in bed watching TV. Ms. Salter was happy to see worker and excited about her Christmas presents. She informed worker that her sister was checking on having her transferred to the Cordova NH because they did not like the Administer, Chester at Shadescrest. Ms. Salter stated she had begun to experience small seizure due to the stress of Kathy Harville, her ex-significant other and Chester. Apparently, the place was in state that the nurse was not able to sit down in his home. There was all kinds of debris on the floors, counters etc. He had a lot of dirty clothes piled up and it was quite unsanitary. The nurse left very emotional due to the state of his trailer/RV. The good news is that Mr. Adams will continue with the DIP program so we can help him that way. In the meantime though, I wanted to see if you were aware of his situation and if there might be anything additional that we could do to help Mr. Adams?
The dilemma is that Mrs. Margie Whitson a patient at Golden Oaks Rehabilitation Center is going through some very hard times after just loosing her son William about a week ago. She has also had to deal with loosing her husband in the past 5 years leading up to this. She is also reflecting back to when she lost her first son to a motor vehicle accident. Margie is having a very difficult time taking this all in and now feels all alone and wants the one and only thing keeping her alive removed. Hi, my name is Caitlyn Blankenship and I am a member of the Northwest Middle School Beta Club. I live at home with my Parents, sisters, and two dogs. I like reading, playing soccer, and serving my community. Sometimes I go to the nursing home and read to the people. I have donated to can food drives and I helped make cards for meals on wheels.
There are a few components that are critical to Mrs.Smith staying safely in her home. The use of a mobile nurse, meals on wheels, and the remote monitoring system are all important. The mobile nurse setting up her remote monitoring system and meals on wheels is imperative. Now Mrs. Smith does not have to risk her safety, or the safety of others trying to drive. She is saved many trips to the grocery store and doctors office, which is much safer due to her deteriorating eye sight. The case manager is also quite important. This monitors how well she is managing her care, and provides her with education about her different conditions.
Luckily, Norma was referred to New Light Neighborhood Community Center and was assigned to Tom who is an experienced MSW social workers. Tom began to engage with Norma by learning about her individual case and what issues she is facing. He can continue this process of engagement by preparing for action with other programs that might fit her needs such as Temporary Assistance for Needy Families, Child Care and Development Fund, and the local County Hospital. He should sit down with her and make sure these will be of an advantage to her and begin to come up with a plan that will suit her and her children.
We were lucky to host Blanka and her wonderful family. We met them upon their arrival in the apartment on check-in day. We talked and talked about Copenhagen, Denmark, USA, CA, Airbnb and a lot of things. Were had to go in order to catch a train to the airport but none of us could hurry up. We have written many positive reviews about our guests. We don’t accept every booking. Especially we prefer families. Well, this review is a positive one as well, and one of the very bests! We can recommend Blanka and Art and their bright and cute kids Jacob and Joshua. We hope to meet them one day again. They left the apartment in the best way possible, very tidy and very clean and everything in perfect order even though they had to leave early in the morning.
