I have been experiencing episodes of fatigue and have required more down time than usual. I have continued to try to work as the withdrawal of the CCP Disability Benefit has caused me financial hardship. I have been pushing myself to work to meet my financial obligations to support my family and myself. UCM: CPSW met with Mr. Mauer for supervision at Brookdale and the visit went well so far. Mom and dad played with their son. Ms. Mauer changed Kaison's diaper and feed him during the meeting. Kaison seemed little tired today. Mr. Stark joined us today and he mentioned that he was in jail for two days and his case was dropped because it was wrong allegation from a friend. Ms. Mauer, Mr. Stark will be meeting their county nurse today after supervsion visit is done. Kaison is doing well and he had no fever and no coughing during the visit. Could you please advise? Appeal# 1090436 for claim# 125117839600 was worked by you on 05/19/2015. You upheld the decision. However, sent out a letter to the provider advising that it had been overturned. We need to know which decision was given on the appeal? If it’s different then the letter that was sent to the provider, another letter needs to be sent to the provider with the correct decision. If the letter is correct them the claims need to be reprocess for payment.

This is an appeals letter for the University Academic Appeals Committee. This is a letter of appeal for the summer Counseling Art Therapy Class with Phyllis Nodler.
