CM spoke to Ms. Quinones (caregiver) in regards to a follow-up on services for Taina (youth). Caregiver informed CM that youth met with IIC for the initial session. Caregiver reports she likes the IIC. CM was informed that caregiver attended the youth’s school meeting on 2/23/17. Caregiver explained to CM that youth teacher reports youth is failing her classes and she continues to misbehave in school. Caregiver also, stated that since 2/23/17 she has not had any phone calls from youth’s school due to her misbehaving in school. CM will continue to monitor youth’s behavior and grades in the school and therapeutic sessions with IIC.
CM spoke to Kelly (Case Manager) and Alexia (youth) via phone conference in regards to a follow-up on youth. CM was informed that youth was able to remove her braces and has been rebuilding her relationship with caregiver. Youth is willing to reside with caregiver after completing program. The team discussed after care services for youth; the team rescheduled CFT meeting for Wednesday, 8/16/17 at 12:00pm to discuss transition plan, in-home services and education needs.
Case management services were provided in garden Grove. Present at the meeting were WYP (Wraparound Youth Partner) Andy Ngo. Service minutes are high due to WYP Ngo attempting to meet the youth. WYP Ngo reminded the youth of the meeting during the after noon through a phone call. The youth acknowledged the meeting and will be waiting for WYP Ngo. WYP Ngo arrived at the youth’s home. WYP Ngo contacted the youth through a phone call, but the youth hung up on WYP Ngo. WYP Ngo waited for the youth to show up. WYP Ngo contacted the youth again, but the youth did not pick up. WYP Ngo contacted WCC (Wraparound Care Coordinator), WPP (Wraparound Parent Partner) Lyneth Torres, and SUP (Wraparound Supervisor) Sheila Gaston-Cruz to inform them the youth
A, B) MSTT met with the caregiver to examine how she plans to increase the supervision and monitoring of the youth while out in public. Caregiver expressed while the youth is at home she is going to speak with her mother and his aunt to help while she's at work. MSTT encouraged caregiver to reach out to the school and ask them if they would be able to supervise and monitor him. Caregiver contacted the school and is waiting to hear a response. MSTT will continue to work with the caregiver in increasing the supervision
CM spoke to Kiara Gelin (youth) regarding a follow-up on services. Youth reported she continues to attend the Partial Hospitalization program at the Jersey City Medical Center. CM was informed that youth stopped attending the TASC program due to time conflict and youth will resume the program in September. CM and youth disused employment; youth had a job interview at Wendy’s, Fast food restaurant. CM encouraged youth to apply at local jobs in the mall, CM verbally provided youth with a list of jobs that are hiring in the community. CM inquired about the Cook, Eat and Talk program. Youth reported the family will follow-up with the program once the family phone is working. CM agreed. CM and youth discussed mentor
On Sunday, January 10, 2016 at approximately 2135 hours, FHEO Security Officers were dispatched to the Special Care Unit (SCU) room E.D. #38 for a Disorderly Baker Act patient attempting to leave. Security Officer Omar Alonso along with Officer Thomas Mejia and Supervisor Steven Evans, arrived at 2136 hours and met with Nurse Kristen Strauch who was standing next to the patient. The patient, Tyler Hull (DOB: 01/07/1987; FIN# 85068604) was in a state of anxiety and stating that he could no longer take being coped up in a room and he needed to leave. Security staff explained to the patient what the Baker Act Law meant and why he was not allowed to leave until he had been medically cleared and seen by a Psychotherapist.
Case management services were provided through a face to face interaction in Santa Ana. Present at the meeting were WYP (Wraparound Youth Partner) Andy Ngo and the youth. WYP Ngo checked in with the youth’s father. The youth is behaving well this morning. The father promised he will play with the youth after the one on one session. The father is concerned about the check for daycare. WYP Ngo will update the father as soon as possible about the check. WYP Ngo took the youth to get his incentive (frozen yogurt). The youth liked the treat. WYP Ngo reminded the youth to work hard toward his incentives. The youth wants to work for shaved snow for the next incentives. The youth had trouble with accepting no as an answer. WYP Ngo ignored the youth
Hi Lora, I liked how you incorporate client-centered approach throughout your initial post. Adding desire goals will increase patient cooperation during treatment sessions. Sharone likes singing and spending time with her family, so in my opinion it will be a good idea to have karaoke every Saturday with her family members. Choosing an easy and favorite song for Sharone could help on her slurs speech, guiding her to hold the microphone with her right hand, will work on right side body awareness/movement and playing don’t forget the lyrics can help her improve her cognition and concentration skills. My documentation will say: Sharone will engage with family members in a social activity with 50% accuracy. She will sing a song of her choice by holding the microphone with right hand. She will start in a standing position for 3-4 minutes, then transition to a sitting position to improve on her standing tolerance.

CM spoke to Jim Warner (IIC) regarding a follow-up on Makaryos (youth) progress with therapeutic services. CM was informed that youth continues to miss numerous days at school and IIC discussed consequences with youth. CM and IIC discussed the truancy program with TYSN; IIC agreed for the youth to do the 12 week program. CM will follow-up with TYSN regarding the program. IIC noted youth did not receive the summer school application. CM encouraged IIC to informed caregiver of this matter to enroll youth into summer school. IIC stated youth went to the Jersey City Medical Center this week to conduct an evaluation and the family has to return next week to finish the evaluation. CM invited IIC to youth’s scheduled CFT meeting
Yes, there are several different types of abuse that the elderly could be experiencing. Many people don’t like to talk about abuse due to fear of retaliation, shame, and embarrassment. It is best to interview the patient and caregiver separately. There could be barriers to obtaining a reliable history such as the patient’s cognitive or psychiatric conditions and family dynamics. From experience I have learned that many caregiver especially when it’s a family member don’t mean to cause harm. Caring for someone is a full time responsibility and it can be very demanding and overwhelming. Often times the person isn’t educated about available resources and is ashamed to ask for help. The room was filled with baby beds that lined the wall. Caregivers took turns feeding babies based off the time that they had previously ate. Some babies were put in a highchair to be fed something other than milk. After feeding the babies the caregivers would write on report sheets what time the baby ate. The sheets were located on the all as soon as you walk in. The were for the parents to retrieve when they picked up their baby(s).
CM spoke to Marcus (IIC) regarding a follow-up on Joseph (youth). CM was informed IIC met with youth last week for 2 hour session. IIC stated youth continues to improve his behaviors in the home and in the community. CM and IIC discussed transition for youth; IIC and CM are in agreement to conduct 1st transition meeting for Thursday, 5/11/17. CM will discuss transitioning with the team. CM informed IIC of youth’s IEP meeting for Monday, 4/24/17; IIC reported he will not be able to attend. CM was informed the family is currently on vacation due to spring recess.
