Case Management : A Nurse For Multiple Years

Case Managers have a challenging job, often dealing with patients, community agencies and facilitating their hospital discharge to the next level of care. They collaborate and communicate with the entire healthcare team and mostly with the patient during the hospitalization process. They act as support for all stakeholders to achieve positive patient outcomes. In this paper, I will be interviewing Ian Mopas, who is a Patient Care Coordinator at Kaiser Permanente in Redwood City about his education, work training, goals, and objectives as a Care Coordinator, and his responsibilities in his organization.

The Case Management program is offered by Kaiser Permanente to support members with complex medical, psychosocial, and care management essentials. This specialized service is provided by case managers. The case managers are either Registered Nurses who are certified in case management or qualified Social Workers. Case managers provide necessary information and education to promote understanding, reduce the chance of possible complications, and facilitate effective and proper delivery of care and services.

The aim of this study is to provide a detailed account of the nursing care for a patient who is experiencing a breakdown in health. One aspect of their care will be discussed in relation to the nursing process. The model used to provide an individualised programme of care will be discussed and critically analysed.

The interview of a case manager is designed to give students the chance to create an opportunity of interaction with case management. During the interview, the focus is to establish connection in understanding the purpose of their duty, adapted theory of function, ability to maintain self-poise, intellectual thinking process to maneuver from one case load to another, and demand of the population to whom the service benefits. The interview process provides an overview adjustment to distinguish the relevance between the provider mission statement in contrast with the employee who transforms the vision to concrete, workable possibilities of wonders while helping the client. The reinforcement of speaking live with a case management employee helps to understand the purpose, the process of initiating help, and placing individuals with community resources to help support individual needs.

The case management process consists of five parts: assessment, treatment planning, linking, advocacy, and monitoring. It is not a linear process and does not follow the order it is written up. There may be some back and forth and returning to certain points.

Careful attention must be focused on the safety and welfare of each client. Interventions for service delivery are to be spiritually and culturally sensitive respecting each client’s individual needs and preferences. The role of the case manager and the various duties they perform requires education, skills and training to address client needs appropriately. Case management is a continuous balancing act of judgment calls, making ethical choices, getting along with coworkers, and following legal protocol. It is the duty of the case manager to know how to develop a relationship with both clients and coworkers while still maintain their professionalism. The healthcare professional must always leave their personal opinions at home and provide the same quality of care to all their clients regardless if they have different views, religion, sexual orientations, or cultures. The case manager needs to ensure that the client’s concerns will be put first no matter what, and should also clarify that the as the professional he or she will do all that is necessary to provide the best quality of care to their client.

It is very important for graduate nurses to fit in to a hospital as this gives them a chance to practice safely and effectively. For a New Graduate Nurse (NGN), fitting-in is about establishing secure and meaningful social bonds with ward staff (Rush, Adamack, Gordon, and Janke, 2014, p. 222). Fitting in, or the wish to fit in with ward staff is one of central piont of NGN transition experiences; it gives the NGN a feeling that they are a part of a social group with common goals, common experiences and a shared culture (Tingleff and Gildberg, 2014, p. 537). Success for the New Graduate Nurse would depend on how fast they establish friendly relationships and a sense of belonging. Hospitals understand the importance of NGN getting used to the

Case managers play such a key role in the health systems that they are almost considered as the face of patient care.

Case management has case managers who effectively assess my healthcare needs. They also provide support systems to help me. Then, they coordinate, monitor, and evaluate my progress and the services that I receive. It promotes quality and cost-effective interventions and outcomes.

The case managers' role is diverse. It consists of screening, assessing, planning, implementing, following up, and evaluating the situation of the foster child assigned. They are essential in the child's case plan by acting as a liaison that connects the foster parent and the state. Besides bridging the gap, they also make sure to facilitate the appropriate resources.

For my Clinical experience, I was referred to one of community clinics run by nurse practitioners - yes, NPs- in Suffolk County in Long Island by my coworker. It is called “Nightingale Preventative Care.” I am working in the ER and at first, I thought this clinic would be a type of urgent care office which is a similar setting to the ER. I was totally wrong. For the past two weeks, this place has surprised me many ways and I learned about what the community clinic is alike to its neighbors. Patients can be seen by NPs by the appointment. However, it is located inside of K-mart and has many walk-in patients as well. Many patients who come to visit for their check-up have no medical insurance. Every Wednesday, a representative from Fidelis Care insurance company comes and provides information about Medicaid and Medicare service the company has. I really like to sit down with patients and assess about their medical histories and family histories which I cannot do often in the ER. I had a patient who was Hepatitis A Ab, Total positive Abnormal first day I work at the clinic. He didn’t understand what the test result meant and neither did I. I printed out an article from National Library of Medicine and went over with him. Patient’s education in the ER rarely happens from nurses. I felt great to listen what patients tried to lose their weight or quit smoking. I like to continue on developing skills on patient’s education and preventative care measure for patients.

The BSN case manager is responsible for collaborating with the patient, physician, RN, CNA, family and other ancillary departments. She is also involved with the more complex aspects of the daily care and establishing a relationship with the patient and family.

Case Managers help their clients contact DCS and support them in court and other meetings associated with DCS cases. The focus of case management in this role is to foster the parent child relationship and support sobriety in the controlled center environment and outside in the world where triggers will happen constantly. Additionally case managers help teach the clients to understand that there are rules that must be followed in order to successfully be a sober parent to maintain custody of their children long

According to IC & RC, Case Management is defined as, “activities intended to bring services, agencies, resources, or people together within a planned framework of action toward the achievement of established goals. It may involve liaison activities and collateral contacts” (Herdman, John W., 6th Ed.). Case management is a concerted effort of various professionals in the human social services network that assess’, plans, implements, coordinates, monitors and evaluates options required to meet the client’s health and human service needs. It is characterized by advocacy, communication, resource management and promotes quality cost-effective intervention outcomes. The Case Management Process centers on the client and the client’s support system. It is holistic in its approach to the management of the client’s individual and specific situation and that of the client’s support system. It is adaptive both to the case manager’s practice setting and to the healthcare setting in which the client receives services. Case management is not a profession unto itself however; it is a cross-disciplinary and interdependent specialty practice within the health and human services profession.
