On August 17, 2015 at approximately 1:27 AM I, Deputy Bowring, was dispatched to 238 Cr 4568 regarding a disturbance. Upon my arrival, I spoke with Wanda English, who stated that her son Randy McLeroy was drunk and busted the windows out of her house. I asked Wanda if she knew where Randy may have went since he had left prior to my arrival. Wanda stated that Randy has a house at 105 Cr 4568 but it does not have electricity. I then did a perimeter check at Wanda’s residence to see if I could locate Randy. While doing the perimeter check I observed several busted window at the residence. Sgt. Cates arrived on scene I advised him of the situation and were Randy possible location could be. I then advised Wanda that I and Sgt. Cates were going down
I then escorted Inmate Staggs to the Durango Medical Clinic. While on the way to medical I asked Inmate Staggs what time he fell in shower? He told me around the time the evening meal was served. I asked Inmate Staggs if the reason he is going to medical was due to a crime committed against him while in MCSO custody? Inmate Staggs said "No." After that I dropped him off at medical. I then went to review the Gentec Security Desk System with Sgt. Salazar B0654.
At 5:30 SA Lyn Brumaire received another call from Officer Tabiri that another resident named Marielle Fisher may know the whereabouts of Ms. Lee. Upon knocking on the door of suite 805, no one answered so officer Tabiri asked me to key into the room. Ms. Fisher was not present but her suite mate, Ms. Neali Rogers was. Ms. Rogers communicated that Ms. Fisher had gone home for the weekend.
On July 22, 2017 at approximately 6:07 PM I, Deputy Hill along with Sergeant Bowring, Deputy Dimert and Deputy George received word that Wayne Jones was at 155 CR 1686 and had a class B warrant. Deputy George had spoken with a complainant on the phone that informed Deputy George that he had seen Wayne Jones in the area and know that Mr. Jones had a warrant for his arrest. After Deputy George informed me of the information I contacted France Young a residence of the area, I asked Mrs. Young if she had seen Mr. Jones and Mrs. Young confirmed that Mr. Jones was at 155 CR 1686. After verifying the warrant on Mr. Jones though dispatch all the above units check en route to that location.
MMCC, a Medicaid managed care organization, is located in the low-income community of Baltimore City. It is positioned across from a major medical center that does not provide its services. MMCC seeks to reduce their medical loss ratio (MLRs). As part of their strategy, they enroll poor people of the community through incentives, utilize a distant medical center to provide their services, and cut back on administrative requirements. To get communities members to enroll, they offer turkeys to enrollees during holiday times. They fail to inform members that they must use the medical center across the city and not the one directly across from their location. Once a member of MMCC, the patients experience poor quality care. Operators are limited and cannot keep up with calls to make appointments. Health care delivery site’s parking lot has limited parking spaces. The waiting room has limited seating. The actions of MMCC are legally and ethically questionable.
On February 7, 2017 at 1538hrs, I was dispatched to the information desk for possible weapons in a vehicle. Upon arrival I met with Veteran Raymond Blanchette - 2671. He had come in to facility to ask about his firearms that were in his motorhome. Blanchette state he lost his house and was living out of his RV. Further RV was his way to get back and forth to appointment. I decided to warn him about the weapons being in his RV. I based this decision on the fact he, had to use walker to get around, being Homeless and being an elderly man. I did inform him if brought the RV with weapons back on station we would charge him with a weapons violation.
On 5/30/18 I met Mr. Reid at the office of Dr. Rampersaud. I explained that the insurance carrier is not getting the form filled out correctly regarding his narcotic medications. I asked his permission to meet with Dr. Rampersaud when they go back to the examination room and leave once we discussed the form. Mr. Reid agreed. He reports that since having the spinal cord stimulator battery replaced his pain is 60% better. He reports his pain level is a 6. He continues to have his legs give out unexpectable. He reports needing help from his wife to roll him over when he is in bed. He continues to use a wheelchair. Mr. Reid said he wanted to speak with Dr. Rampersaud regarding decreasing his medications at least for the summer. He feels the warm weather makes his pain more tolerable.

On average Camden is faced with a homicide occurring roughly, every 33 hours. Children and families within the community prefer to stay inside their homes and are terrified to be outside in certain areas (adamv2).
I responded to Magnolia Wood Lodge in reference to a dirorderly call. Upon arrival, I made contact with Nathan Mosley. Mosley stated that one of the residence(Cedrice Barnes) was being very un cooperative and attempting to hit them with closed fist. Nathan Mosley, Angela Lewis and Cessiley Colwell, were all victims of the situation. Officer attempted to walk Barnes back to his room. Officers ended up getting Barnes back in his room, which was room 11. Officer's then talked with Barnes and explained the issue with him. On 3/17/16 at 12:10 A.M, Safety Specialist Kevin Sabulsky notified Shift Supervisor (S/S) Enmanuel Cabrera that he saw the officer who was positioned in the exit shack not check the back of a trailer. S/S Cabrera advised Safety Specialist Kevin Sabulsky that he will look into the matter. Upon further review of the video surveillance system, S/S Cabrera saw that Security Officer Dinh Nguyen did not check the back of one trailer at 11:57 P.M on 3/16/16. S/S Cabrera immediately made his way to the exit shack to speak with S/O Nguyen. On 15 October 2017, at 1757 hours, Deputy Tabor and Deputy Williams were dispatched to 135 South Penalosa Street in Penalosa, Kansas, Kingman County, in response to a disturbance. As Deputy Tabor was en route, he was advised by dispatch over the radio that there were young kids who had threatened to "off" the reporting party.
I was waved down by a female at the entrance to the Edge Apt. Alize Rivera advised that she had been involved in a motor vehicle accident on Elm St. she stated that the other driver involved had her boyfriend the stated driver of Unit 2, move from the scene of the crash to the Edge Apt, and then he drove off. Rivera advised that the driver of Unit 2 ,was stopped waiting to turn into the Troy Softball parking lot when a black Chevy Silverado struck her vehicle from behind. She stated that the Driver of unit 1 was a white male, around his mid 20, and had a blue hat on. Officer were unable to locate unit 1. Unit 2 had major damage to the rear of the vehicle ,but was not disabled. I advised on how to obtain this
On 07/09/17 I contacted Sergeant Starks by telephone at shift change in reference to the daily pass on. Sergeant Starks advised that he was en-route to Deputy Hollis who had been exposed to OC Spray while on a traffic stop. I obtained his location and proceeded to the area; I-440 @ Mile Marker 11.
After having a long discussion i went back up to headquarters a had witnesses had called me telling me that they have information they wanted to give me.
