At approximately 10:03pm after arriving to the community base home from Elizabeth NJ, Mr. Taylor observed Student George running into the Haddon house in a violent manner. Mr. Taylor then entered the home and observed Student George using profanity and throwing the television on the floor. While approaching the student Mr. Taylor prompted the student with several concern verbal’s. Student ignored staff directive and immediately ran out the front door. At this time staff exited the Haddon Home to look for the student. Unknowingly to staff the student ran around back and kicked the door into the Collings House in attempted to physically harming another peer Once Mr. Taylor entered the Collings House he observed Student George throwing a chair
Dionnie Williams & Jaziaha Lawson are placed in Ms. Holland home. The home is not a pre-adoptive home and Dionnie and Jaziaha have expressed that they do not want to stay at their current placement long term. Dionnie and Jaziaha were in respite care from August 12, 2016 to August 14, 2016 at foster parent, Bashera Grove home. Jaziaha and Dionnie have both expressed to me they would like to be placed in Ms.Grove care long term. Ms. Grove is a pre-adoptive home and is interested in keeping the girls long term. I believe it would be best to place Dionnie and Jaziaha in Ms. Grove home before the start of the new school year. Please let me know what your thoughts

On this date worker received a call from Tori Pettit. Ms. Pettit stated that Serv-Pro had visited Ms. Smith's home did an assessment and sent off the mold samples. The results came back that the house is not livable due to mold infestation. The mold is in the walls, ceiling, basement, and air vents. Ms. Pettis has spoken with Ms. Smith and Ms. Rachel Rodgers about Ms. Smith moving ASAP. Ms. Pettit stated that Tri-County could place Ms. Smith into a group home now due to her home being deemed unlivable. However, Ms. Smith is not interested in-group home placement and would rather live independently. Ms. Rachel Rodgers has agreed to assist Ms. Smith with apartment applications and any other thing need in order for MS. Smith to find residence.
A report was received on 06/05/2017 alleging that the mother (Hermionne) left Ashante (C-V 17) with a non-relative since 02/2017 without any legal rights. According to the report, the mother refuses to take her child back home and will not engage with Ms. Aarons (caretaker) to provide legal documentation for the child to be enroll in school and taken to a Primary Care Physician. Ashante has not been is school for the past 4 months and are unable to enroll in school without paperwork. According to the report, Ashante self mutilates her arms, and the mother refuses to get counseling. The report indicates Ashante was to follow-up with a Cardiologist for a chest pain and the mother refuses to take her to the doctor.
On February 3, 2016, Hakeim was before the Portsmouth Juvenile and Domestic Relations Court for charges of grand larceny, and assault and battery on a law enforcement officer. The charges were taken under advisement of the court with a review date set for July 7, 2016. He was placed on indeterminate supervised probation, ordered to counseling (completed Job Readiness and Anger Management) and a substance abuse evaluation. At the time, the youth and his mother resided in the city of Norfolk, Virginia. Courtesy supervision was accepted by Norfolk Court Service Unit on February 24, 2016. According to reports, Hakeim’s adjustment to supervision was problematic in that he displayed behavioral problems in school and had poor attendance resulting in a 10 day school suspension and a house arrest sanction. DFCS worker accompanied Investigation Terry Smith to 317 West Main St, Sardis, MS due to reports coming to the Sheriffs Department alleging that Dakota, his girlfriend Kynli, and Dalton are staying in a home alone with no adults. The concerns individual also stated to Invest. Smith that the children were not in school. Kynli is pregnant and will be having her baby within the next month. After going to the home Invest. Smith and DFCS worker were able to assess that the allegations were true and the home was filthy and no adults were in the home and the children were not going to school. Client Michael R was picked up at Silver Saddle bus stop at 6:45am in transit to Sinagua Middle School, he got off route 11 to waits 20 minutes for route 88 which is his transfer bus to Summit High School. While waiting for his transfer bus he was approached by another student that confronted him about a situation that transpired with Michael and another student that goes to Sinagua. Student made threats to Michael’s family and Michael attacked the other student. Silver Saddle was contacted by assistant principle Mr. Howard who informed staff of the situation and that Michael refused medical care. Due to student oppositional and suspicious behavior and Officer Lee concerns about student possibly having drugs on campus we asked student to search his bag. Student became more defiant using profanity towards Officer Lee and I, and saying “You are not going to search my bag.”
The reporting party (RP) is group home youth Laura Qi DOB: 4/11/03 DOP: 9/22/16 SCHOOL: Clyde L Fischer Middle, 1720 Hopkins Dr., San Jose CA 95122-1632 SW: Irene Lee (408) 501-6650. The reporting party (RP) stated she has been bullied for the past 4 months by both the group home youth and the staff members. According to the RP she has been called names, physically hit and punch by other group youth, and threatened with physical harm from staff members. These incidents has causes the RP to feel unsafe and place her at risk for harm in the group home. On 1/22/17 the RP disclosed she was hit by another foster youth and reported the incident to a staff member named Dan McGirt, however, Mr. McGirt didn't believe her and stated to her "you are full
Youth is currently attending Dieruff High School. Youth has a history of not attending school. Youth struggles with assignments in school, which is hurting his GPA. Youth struggles with a negative peer association. Youth struggles with controlling his anger and it lead to him getting into an altercation with a sercutiy guard at school. The incident cause the youth to be detain and placed on probation.
The case that we are evaluating is about two high school student who were involved in a physical fight, which led to one student being arrested. The students who were involved in this altercation were Jamal a junior who was known to be a student with good grades and Juan who was a troubled kid who was a recovering alcoholic. Juan received poor grades in school and was known for having many absences. Juan was held back in sixth grade and struggled with social situations. He had never been involved in an altercation before.
On March 1, 2016, I responded to T.C. Williams High School and spoke to Ms. Bailey as the School Resource Officers were not in school grounds. CCIB received a telephone call with an allegation against the facility. RP called to report that there is drug use and domestic violence happening in home due to the licensee runs a daycare. RP reported that on 10/9/2017 Veronica and Juan went to pick up children from school and Veronica asked staff at the school to call the police. RP reported that staff told her to contact the police to report what is happening. The RP reported that the police came to the school and arrested Juan in the parking lot. RP stated that it was stated on the police report that Veronica was hit in the face and kicked (unknown when incident happened). The reporter was contacted at 2:08pm at 09/01/2015 for additional information. The reporter said they were suspecting abuse, but didn’t specify. The reporter was asked was she suspecting molestation, and she replied that she didn’t want to say something that wasn’t true. The reporter never gave a clear answer to what she was alleging. The reporter said Jessica (teacher) has been very concerned about Harmony. Harmony transferred to Mooreville Elementary on 08/24/2015, and she has been arriving to school late (10:30am). It is unknown how many times the child has been tardy/late. The teacher (Jessica) spoke with Wendy about the victim being late for school, and she said “the child will be on time and they were in the process of moving”. The child has been coming to school not clean (not getting a bath), and Jessica felt as though something was not right. Also, last night (08/31/2015), the reporter said the child did not get a bath.
