Union County stakeholders participated in a Health Connections Council project to conduct several caregivers focus groups. As part of a county needs assessment, the focus groups were to document caregiver needs. The primary issue identified during the assessment was lack of case management support to caregiver programs and services. As a result, a caregiver support coalition is being developed in the city of Elizabeth.
In the U.S, one in four will be aged 60 years and older by 2050 (U.S. Census Bureau). This represents an overwhelming number of people who will either be in the caretaker role or be the ROC. Like today, most of the care will be provided by informal unpaid caregivers. The number of informal unpaid caregivers is expected to rise from 20 million in 2000 to 37 million in 2050 (Office of the Assistant Secretary for Planning and Evaluation [ASPE], 2003). Because of the burden of care giving, many caregivers will experience depression, poor health and quality of life (Etters, Goodall, & Harrison, 2008). Their well-being is an important public health concern.
Carmel Health Network (CHN) is a Christian, private, non-profit, 501(c) (3) corporation founded to deliver high-quality primary health care to children living in the inner city of Mobile, Alabama. CHN provides access to primary healthcare for underprivileged children of Mobile, Alabama (and its surrounding area) as well as empower, educate, enlighten, and inspire youth to attain goals that may appear insurmountable (www.carmelhealthnetwork.org).
Partners HealthCare is a non-profit, health system located in Boston that created a data based transformation (Davenport, 2013). It integrated a new system that aligned the participating organizations to cohesively run as one and to help shape the future of the organization. The system didn’t stop there as it was responsible for bettering the patient financing experience and the delivery of healthcare information to other organizations (Davenport, 2013). The initial goal of the organization was making patient care more affordable and accountable by providing integrated, evidence based, patient-oriented care.
The number of seniors in the United States is growing rapidly. The costs to cover medical expenses is increasing as well, causing many family members to become caregivers. Unfortunately, without receiving specialized training, some seniors may be at a disadvantage when they receive care from their family members. Caregivers must recognize the signs many seniors exhibit when it is time for a community that focuses on senior living in Virginia Beach.

Care 2 Caregivers is a peer support line for caregivers to strengthen the caregiver’s network. Services provided include peer support, counseling, health education, disease management, and referrals to community agencies for support and assistance. WellCare partnered with Care 2 Caregivers and handled 2,200 individual cases, and had 8,026 contacts with caregivers. Nearly two-thirds of the contacts provided peer support or counseling services.
When your senior loved one needs care, it is not uncommon for loved ones to rally around and provide support. In fact, the Centers for Disease Control estimate that there are nearly 35-40 million senior caregivers in the United States. Of these caregivers, 90 percent of them are family members and loved ones. Despite our very best intentions, however, not everybody can provide all of the assistance needed for their senior loved one. In these circumstances, in-home caregivers become essential.
Millions of people are in the role of a part or full time caregiver in the U.S to family members who have a serious illness or disability. This a a very challenging and demanding role that needs to be recognized and appreciated.
The HealthConnections Model is our investment into social innovation. Our model encapsulates four complementary elements to address social determinants of health. The first tracks community-based health and wellness events so we can provide this information to meet community needs. A second element establishes HC Councils to pool resources with local social service agencies, field advocates and community leaders to identify care gaps in the social safety net, and make informed decisions to address these needs. A third element involves an Investment Program to provide community support in the form of sponsorships, grants and investments (partnerships) to fill important social service gaps. The policy that will be analyzed is the federal Older American Act Title III-E, which was approved by the Congress in 2000. It regulates National Family Caregiver Support Programs (NFCSP), a categorical non-income-tested program, aiming to provide direct services to informal family caregivers. Under the federal administration from AoA, the NFCSP is carried out through a network of 56 State Units on Aging (SUAs), 655 Area Agencies on Aging (AAAs) and local public or private institutions. Specifically, AAAs provide five-category services to caregivers by using the allocated NFCSP funding—information regarding services, assistance for accessing services, counseling/support groups/training, short-term respite and supplement services on a limited basis.

The Center for Connected Medicine (CCM) has not made any updates to its digital marketing in over 20 months. This is due primarily to drastic staff changes. In addition approvals for marketing in the format of a joint partnership, such as the CCM, can dramatically slow down the marketing process.
The National Family Caregiver Support Program (NFCSP) was established in 2000 and “provides grants to states and territories” “to fund a range of supports that assist family and informal caregivers to care for their loved ones at home for as long as possible” (AOA, 2012). Some statistics on caregivers are that twenty-two percent of caregivers care for two individuals, while eight percent of caregivers care for three or more individuals (AOA, 2012). Interestingly almost half of all caregivers are actually over the age of fifty years old, which makes them more vulnerable to their own decline in health (AOA, 2012).
