On March 23, 2017 at 1402hrs, Doctor from Mental Health notified Police Service of a Health Welfare. At 1410hrs, VA police notified the Austin Police Department (APD) and spoke with a dispatcher – 7974 and requested a Health and welfare at 2407 S. 4th street (Green Doors Halfway House) for a possible suicidal Veteran. APD Dispatcher stated he had officer’s en-route to the above address and provided me the incident #170821057.
Once Dy. Cavalier returned to work on Saturday May 14, 2016, he was again assigned to M&N control. Half way through his shift he was advised by Cpl. LaMotte that he needed to contact medical and asked them if they could come and check on Ducre because he was acting strange. Dy. Cavalier contacted medical and spoke with nurse Sterling and she advised him that she was eating and that she would let the others know and someone would come. For the remainder of the night Dy. Benson made extra rounds to check on inmate Ducre. Upon the time that Dy. Cavalier’s shift ended no one from medical had seen Ducre.
He was brought in by his friend Gavriel Martinez. At 2338 hours Orange County Sheriff's Deputies Kenneth Jorgensen (Badge# 8180) and Deputy Michael Fairley (Badge# 6210) arrived and interviewed the GSW patient, both deputies were already on site for an unrelated event. Security Manager Richard King was informed by text of the incident at 2340 hours as well as Nursing Supervisor Debra Reilly. At 2350 hours OCSO Deputy Kristine Helms (Badge# 5021) arrived and requested to speak to the patient’s friends who was waiting in the E.D. lobby. At 0010 hours Deputy Michael Fairley contacted SO Alonso and advised that it was OK to open the Emergency Department. The lockdown was lifted and Security Manager and Nurse Supervisor were informed of the incident. The patient, Lopez was airlifted to Orlando Regional Medical Center (ORMC) at 0030 hours at which time OCOS Deputy Kristine Helms informed me that the Gang Unit task force has been called for further investigation. While enroute I learned Trooper Dingman was out a 30 Marie Ave investigating. I arrived and found Trooper Dingman speaking with a male & female inside of apt. #1. Trooper Dingman briefed me there was an altercation between Linda Showalter, Jessica Goose, Eli Raisch, & Valerie Hartmann. Trooper Dingman stated there was a call for service about 46 hrs. ago where Valerie reported a family member of Jessica’s was inside Valerie’s cleaning when she was accused of stealing pain medication. Valerie & Linda then confronted Jessica & Eli about it, and an argument ensured.
At 1005 this clinician made contact with the patients CPS caseworker who reports he was scheduled to meet with the son at his daycare. He reports he met with her roommate who made him aware the patient was taken away from the home by ambulance. In addition, the roommate also reported
On December 11, 2016 at approximately 1330 hours, Detention Officer Harden reported duty and was assigned to work 1200 Baker Street Jail, 3rd Floor, A-Pod Control Center. At approximately 1420 hours, Detention Officer Harden advised she was not feeling well. At approximately 1430 Hours, Detention Officer Harden had her blood pressure checked in the 1200 Jail Clinic and was advised it was elevated. At approximately 1440 hours I called for Houston Fire Department. At approximately 1445 hours Houston Fire Department Medic 17 arrived at 1200 Baker Street Jail. Detention Officer Harden declined transport at approximately 1500 hours. At approximately 1505 hours, Sergeant Chad King took Detention Officer Harden to her husband’s work location Lieutenant
On 02/06/2017, at approximately 1715 hours, while I was conducting meal distribution and ID Headcount in Durango Jail, 3225 W. Gibson Lane, Phoenix, AZ 85009, Housing Unit D-3, Pod D. When Inmate Peterson, Jordan T324870, cell 57-2 received his meal tray, I noticed he had dried blood stains on his shirt, his left eye, left cheek and left corner of his lip were bruised and swollen. At that time I asked the inmate what happened and he replied, "It’s nothing." I told him to step out of the pod and have a seat in the multipurpose room of the housing unit.
On Thursday 12/24/2015 at approximately 2307 hours. Security Officer Omar Alonso (420) was contacted by E.D. Charge Nurse Sharey Selover about an uncooperative intoxicated male patient, Jose D. Gonzalez (DOB: 03/30/1977; FIN# 85006354), come in through the EMS Offload area. Officers Alonso and Ayuso reported to the call and observed an intoxicated male being wheeled into the Special Care Unit (SCU) E.D. room # 39. According to his assigned Nurse Sara Lopez, the patient had been involved in a physical altercation and had been kicked hard in the groin area. Patient did not behave badly or disruptive once he saw that Security were present and his Nurse was able to get his vitals, blood work, and urine without having any issues. Security staff
Reporting party (RP) called CCIB to cross report that resident Gerald Gilliland (DOB: 4/22/41) reported that while he resided at this facility, unknown male staff locked him in a room against his will. It is unclear what room the resident was locked in because resident was unable to convey any details about the room. Resident then stated that it was all of the nursing staff who locked him in the room. Resident was hospitalized at Kaiser South Sacramento on 9/18/16 through 9/22/16 and is currently residing at Eskaton Greenhaven. RP stated that it is unclear if the resident has Alzheimer's or not. RP stated that the original reporting party is Sacramento Sheriff's Department (Report#16-286176) and RP will forward the SOC 341 to CCIB.
On 10/21/2015 SO EMT Perez was dispatched to HS-318 regaurding a general assistance call. SO EMT Perez knocked and announced his presnece at the door and was verbally greeted in by the resident, a Mrs. Netha Hopson. Mrs. Netha Hopson stated that she needed help retrieving her purse because her motorized chair was caught on it and she could not get it untangled on her own. SO EMT Perez managed to untangle the purse and retrieve it for Mrs. Netha Hopson. After insuring that Mrs. Netha Hopson needed nothing else SO EMT Perez cleared the scene without further incident.
Upon arrival at 829 Capp St., I made contact with (R/1) Daniel Chavez in the lobby of the residential home for the mentally ill regarding a missing woman. Chavez reported that a patient of his (M/1) Helen Fernandez had gone missing around 1000 hours while in route to an appointment at Sunset Mental Health located at 1990 41stAve., San Francisco, CA 94116. Chavez stated that Fernandez was taking the Muni 48 Line to her appointment and had an active Muni Clipper Card. Chavez reported that Fernandez was scheduled for an appointment at 1130 hours with Dr.
On Monday January 4, 2016 approximately 11:40pm Stephen came downstairs looking for EMS for his mother who was having an epileptic seizure. C. Horton went over the 900 side to see what was going on. C. Horton saw Ms. Sophie lying down on the bed. EMS Gonzalez #4333 unit 39 victor arrived on seen at 11:35pm on the 900 side. Aisha was notified about the incident and also approved that Stephen can stay with his siblings overnight. Ms. Sophie was taken to Brookdale
I, Cpl. Lessane, along with Deputy Ayer, responded to Hampton Regional Medical Center, to assist Deputy Smalls, with a runaway mental patient. Upon arrival, Deputy Smalls stated a mental patient, Travis Johnson, ran away from the Emergency Room with a set of black handcuffs on one of his arms. Deputy Smalls stated he wasn’t sure of Travis exact location at the time. Deputies along with assistance from the Varnville Police Department and the Hampton Police Department patrolled the area in an attempt to make contact with the patient. No contact was made at that time.
Reporting party resident Michale Dean Lopez (DOB: 11/3/58) stated that approximately 2 months ago, that he was lying down in his bed and felt something behind him. Resident stated that he jumped up and turned around and resident Chris (last name unknown) was standing at his bed side with his boxers down and his "thing" sticking out. Resident noticed that his boxer were down half way. Resident does not know for sure if Chris penetrated him but stated that he felt funny and that he takes Ambien along with Trazodone. Resident reported this incident to staff Zack who told staff Nancy. Resident wanted to contact law enforcement but Nancy told resident not to worry and that everything would be okay and to speak with Mr. Beasley. RP stated that he
On Thursday 07/21/2016 at approximately 2223 hours, Security Supervisor Steven Evans was contacted by Assistant Nurse Manager Robbie Philips via landline and asked to conduct a (44V) Enforcement Escort Visitor Off Property for the discharged female patient in E.D. #48. The patient, Susan Harris (DOB: 03/22/1952), had been quarrelsome and refusing to leave. SOs Christopher Paz and Ariel Weiland responded to the scene. Upon arrival, we observed the patient laying down on her bed, we approached the discharged patient and spoke with her. Mrs. Harris agreed to leave without further incident. Security staff escorted Mrs. Harris outside of the E.D. lobby at which time she requested to stay in the lobby till 0530 hours. No incident occurred during
