We received your grievance request regarding Bayada Home Health Care advising you they are only allowed to treat home bound patients and a Nurse refusing to leave your wound care supplies at your apartment leasing office. This request was received by Blue Cross® Blue Shield® of Arizona Advantage (HMO) on May 24, 2017.
We have investigated your concerns regarding this issue. Based on our investigation, our office contacted Bayada Home Health Care and spoke with Lisa a nurse director. Lisa advised us their nurse contacted you by phone on May 22, 2017, and set up an appointment for home care services on May 23, 2017, at 1:00pm. Lisa stated the nurse showed to your home at 1:00pm as scheduled and there was no answer at the door, Kathy proceeded
Initially we had difficulty obtaining his discharge orders as the hospital care manager replied she had forwarded the documentation, but neither I nor Mrs. Bianco had received a copy. Mrs. Bianco made a visit to Mills Creek and met with Mr. Smith at that time she was made aware of an appointment that Mr. Smith attended. Mr. Smith attended a physician appointment on June 28, 2017 with Dr. Leonard, Orthopedic trauma. Mills Creek assisted with the transportation and a mobile x-ray unit was coordinated to obtain x-rays of the femur at the facility as Mr. Smith didn’t bring his slide board to the appointment and couldn’t get onto the radiology table.
CCIB received a Corrective Plan of Action (CAP) detailing the visit to the home on 10/18/17 by Service Coordinator, Sally Cano. Ms. Cano arrived at the home, but no one was there. Ms. Cano proceeded to check Delta Home 3 (located next door) and found 2 of Delta Home Care IV residents along with residents from Delta Home Care 3 at the home. As Ms. Cano was leaving, she observed a bus arrive at the Delta Home Care 2. The bus driver reported that there was no staff at the home and that the residents were waiting outside. It was observed the Delta Home Care 2, 3, and IV to be lacking staff. On 10/19/17, RP and Quality Assurance specialist (QAS) Jesus Ozeda went to the facility to observe the staff to resident ratios and to collect the staff schedules
Housing Update: client NY NY I, II was approved. Client is waiting for DHS manifest to tour apartment. Another alternative housing is MRT once client SSI is approved. Client also mentioned she signed up with Brightpoint Health Home Health Services since 5/9/2015, Client report she will like to sign up with CAMBA/Home Health and she provided BrightPoint Home Health approval letter for CM to submit to CAMBA/Home Health Coordinator. Client is waiting for her coordinator at Brightpoint to return from vacation to close her case, so that she can sign up with CAMBA/Home Health
The Paradise Facility Home Care Inc. is a residential group home located in the historic town of Naples, Florida inside Collier County. The consumer was intact functional, he understood about his condition however, was a little confused about his discharge date from the the hospital. The consumer did not understand why he was not able to go to rehabilitation home. The writer and social worker at the hospital explain to the consumer that because he is able to take care of himself does not qualified him for rehabilitation. However, the social worker did refer the consumer to Mollie Care which is an AFC Home on 1485 Holmur in Detroit, MI. According to Nadra social worker at the hospital the consumer is schedule to meet with Latina Kaigler to do intake assessment with the consumer. The writer did go and look at the home on Holmur St to make sure it is suitable for the consumer.
SC met with Pa, for scheduled RA HV in Pa's home on 03/1/2016. The completed both the LCD and CMI assessment. Pa appeared clean, appropriately dressed in PJ's and sitting on the couch SC in the home. Present in the home were the Pa visiting nurse Scott from Millennium home care. He was on the phone trying to order the Pa medicines. Also Pa’s aide was present from the agency aide. Pa appeared weak and in discomfort. The SC asked Pa if she okay and she reported that she is in a lot of pain. The SC asked if she is taking her medications and she reported that she ran out that’s why the nurse is helping now. The SC asked the Pa how did that happen and she stated that when she saw her PCP in January for her post hospital and rehab discharged he

RP stated that his mother has resided at this facility since 2015. RP stated that since the facility has changed ownership (end of 2016-beginning of 2017), the services are not being provided to the residents. RP also stated that there has been a shortage of staff which has affected the care of the residents. RP stated that he visited his mother on 4/24/16 and noticed that her teeth were not brushed. RP also stated that the facility has hired an outside agency to provide the care giving for the residents. RP stated that the employees of this agency are unaware of the resident care plan. It was reported to the RP by his mother's roommate's daughter, Mindy Kaplin, that on one occasion, she found his mother in the closet scared and crying. Mindy

On 12/14/2015 SC received call from Beninati home care RN from Mercy Home Health stating that Pa is refusing to go to any of her post hospital and rehab follow-up visits.
