The nursing home is not willing to readmit Mr. Uptain and finding NH placement has been difficult due to his behavior issues. According to Ms. Toney Mr. Uptain is verbally aggressive, encourages other patients to misbehave, smokes cigarettes in nonsmoking area, rude to other residences, and waits on female patients outside of bathroom. She had sent out over 100 referrals and has received 65 no, three pending, and 32 no response. On this date worker received a call from Ms. Val Wilson, RN at Alacare HH. Ms. Wilson has been Mrs. Brown’s nurse and been in the home multiple times. During Ms. Tara Brown time as caregiver the home was clean, food cooked, and Ms. Brown received her medication. According to Ms. Wilson Tara moved in during the summer after Larry Brown, son of Ms. Brown locked Ms. Brown out of her home. Tara told Ms. Wilson that her father, Larry Brown was MR and suffered from Schizophrenia. She also reported seeing bruises on Mrs. Brown’s arm where Mr. Brown would grab her. According to Tara Mrs. Brown received money after the passing of her husband Mr. Brown. However, Mr. Larry Brown got the money, spent it, and is now talking about filing bankruptcy. Due

She has participated in a video conference with Marquel since his commitment. She is aware of his placement in the Virginia Beach CPP Program. She has not visited him at the facility, but she has had contact with him via phone. She is currently unemployed. The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.

The consumer report during his assessment that he would like to have income, go back to WCCC and have a place of his own. Reporting party (RP) called CCIB to cross report that resident Gerald Gilliland (DOB: 4/22/41) reported that while he resided at this facility, unknown male staff locked him in a room against his will. It is unclear what room the resident was locked in because resident was unable to convey any details about the room. Resident then stated that it was all of the nursing staff who locked him in the room. Resident was hospitalized at Kaiser South Sacramento on 9/18/16 through 9/22/16 and is currently residing at Eskaton Greenhaven. RP stated that it is unclear if the resident has Alzheimer's or not. RP stated that the original reporting party is Sacramento Sheriff's Department (Report#16-286176) and RP will forward the SOC 341 to CCIB.
The reporting party (RP) stated that 92 year old resident Delora Lovelock DOB: 1/15/24 was residing in the facility from 5/15/2015 to 6/4/2015. Prior to admission the facility was informed that the resident had a broken femur and could not bear weight. The resident required two caregivers to transfer. Initially the facility assessed the resident and accepted her into the facility. Subsequently the resident was not provided the care necessary. The resident was transferred by one caregiver that resulted in the resident's trip to the emergency room where she was diagnosed with contusions to her ribs. On September 21, 2012, ACEDS Case Action Update/Display documented by Social Service Representative, Richard Gordon stated that Ms. Williams arrived to the agency to apply for Supplmental Nutrition Assistance Program (SNAP) benefits, and Medicaid for her child only. During the visit, CARE interface revealed that Ms. Williams had an open Medicaid case in Maryland (#47403882500). Maryland Social Services also reported that she was employed by CPR Medical as of August 9, 2012, and paid bi-weekly $850.00. Ms. Williams also reportedly received unearned monthly income from SSI in September 2012 in the amount of $440.34, and in October 2012, in the amount of $370.54. As a result, Ms. Williams was denied expedited Foodstamps pending paystubs and her application was marked as "pending."
On this date, APS supervisor talked with worker at Amedisys HH in Mobile Co. Amedisys called to report Mr. Parker was left alone in his caregivers apartment since Saturday and she does not plan to return for two weeks. According to Amedisys Mr. Parker is receiving speech therapy, physical therapy, and nursing services. The apartment is two levels and his bedroom is upstairs. Yesterday they believe he took the wrong medication and overmedicated himself and the home was smoky where he tried to cook food. He is also isolated in a dangerous area with no phone service. According to HH, they have called Ms. Nash, the caregiver, three times and she has not returned any calls. APS supervisor referred Amedisys to Mobile Co.
On this date, Matthew Mitchell APS supervisor conducted a meeting on behalf of Mr. and Mrs. Beaird well-being. In the meeting, Mr. Mitchell presented Mr. Joey Beaird and Mrs. Julie Johnson with solutions that would allow Mr. Gail Beaird to return home. However, both Mr. Joey Beaird and Mrs. Johnson declined to take care of their parent independently. Each stated their home was not handicap assessable and they had jobs and could not dedicate that much time to their parents care. The other option discussed was hiring another service besides Visiting Angels since they are refusing to continue services if Mr. Beaird returns home. However, this is the only service offered in Jasper. When the discussion of Mrs. Johnson and Mr. Beaird hiring sitter independently they stated they had tried this before and it never worked out.
CCIB received SOC 341 reporting that during a meeting with the resident, James Brown (DOB: 09/18/84), social worker from Solano Adult Day Healthcare Center, the resident stated that staff Marco has hit him on multiple occasions. During the conversation, Beekman asked HILL to clarify her role and responsibilities as a service facilitator, in addition to the role and responsibilities of HARRIS, LaFrance as the Employer of Records (EOR) for Medicaid Recipient DANIEL, Rose and MCGHEE, Inocencia as DANIEL’s aide. She was also asked to provide any documentation of training on timesheet submission and approvals that she provided HARRIS and MCGHEE under the Department of Medicaid Services (DMAS) Consumer-Directed care aide program.
On this date worker interview, Mr. Ronald Darty, son of Ms. Linda Darty for the purpose of conducting a PARN interview. When worker arrived Ms. Darty was in a hospital, bed located in the living room and did not respond to worker. However, worker and Mr. Darty exited the room and the interview was conducted in a bedroom located in the back of the trailer. Mr. Darty stated he is 42 years old 4/29/75 and has lived with his mother for 42 years. He promised her when she got sick he would never put her in a nursing home. However, recently he isn’t able to provide the level of care she requires. Mr. Darty stated she has a bedsore on her back and when he changes her or rotates her, she screams out in pain. On this date worker received a call from Ms. Kimbrell, Medicaid Caseworker. Ms. Kimbrell stated Mr. Duncan's nephew who is living at the residence in Curry would need to fill out a Statement from Dependent Relative/Community Spouse form. Because he is living in a home, Mr. Duncan is part owner. She also stated Mr. Duncan would be denied institutional Medicaid because he was not transferred to a long-term facility. However, he could receive hospital Medicaid because he was in the hospital longer than 30 days. The transfer penalty does not apply to hospital Medicaid but if Mr. Duncan ever applies for institutional Medicaid, they would need the person who purchased the home from Mr. Duncan to provide something showing the amount of money he spent
