MSC went to Ernest's IRA located on 40 River Road, New York, NY for a face to face visit. MSC greeted staff and staff stated that Ernest was aware that his MSC was coming for a face to face visit. But due to behavioral concerns, he screamed at staff and told them they are wasting his time. MSC spoke to the Residential manager and she stated that they are having several behavioral issues with Ernest. He hasn't been cooperative with staff; he has been non-complainant and refusing to show/attend medical appointments. D-This writer met with the patient as he was placed on HOLD to address the status of the IOP. The patient provided this writer a paper that was provided to him with listing of IOP for him to explore. The paper shows scribbles of the patient taking down notes about his attempts of who he called. The patient reports Connecticut Addiction Recovery will call him back within 24-48 hours. The patient was able to schedule an appointment with New Direction for May 20th at 7pm; patient spoke with Dan. This writer commends the patient for all of his efforts; however, the patient needs to schedule something earlier than May 20th. This writer asked the patient about ICRC-Coventry House. According to the patient, he called the contact number and showed proof. The patient reports that ICRC gave him two different number and told him to do a walk-in at 8:30am. He reports the patient’s roommate was subsequently evicted from his home after the landlord inquired about the ambulance visiting. He reports he contacted her father concerning the patient residing within the family home, but the father has said no. In addition, he reports he has attempted to contact her uncle but has been unable to make contact with him. He reports he has attempted to make contact with her said friend who is considering allowing her the opportunity to reside with her but she has not answered her phone and he has been unable to leave a voice message. He reports no one wants her in their home, and the patient has “burned her bridges” with family members. He reports her family would benefit from counseling. CCIB Intake received a call from resident Eugene Kunz DOB 2/8/24 in room #6. Mr. Kunz call to state he wanted to remove his daughter Joyce as his Power of Attorney (POA) and pay his $3000 rent each month. According to the caller he has residing in the facility for approximately 3 1/2 years and wanted to return to his home, however due to his slight dementia he was having difficulty with his memory and therefore could not return to his home. Conversely the caller described how his daughter would have him examined by physician who would give him 3 words at the beginning of his examination and at the end would ask him the 3 words. Unfortunately the caller could never recall the 3 words and thus was unable to return to his home. Recently the caller
D-Met with the patient to address a fax from DCF request of the patient records. The patient immediately said, " Don't send this shit. I cannot stand that worker.....I was in court the other day and I am tired of this and this worker always in my business." The writer explained to the patient that should he continue to refuse to sign an ROI, it may hurt his reunification with his son as the patient detailed to this writer about how DCF got involved in his life ( According to the patient, he was intoxicated when visiting his son and a case was called against him and the child's mother). The patient made it clear to the writer that he has no desires to signed an ROI for DCF and for the clinic to ignore DCF request.
On 7/11/2015, CM did a visual and had client come to the social service office. CM completed Bi-Weekly ILP Review. In the meeting client appears to be wear out, and tired. She was constantly throbbing her forehead, like if she was having headache. CM inquires what the problem is. Client replies “she doesn’t like the shelter food and sometimes she doesn’t eat” CM advised the client to eat and nourished her body. CM also observed that client is depressed but she continues to refuse medical referral to see a psychiatrist and medical doctor. Client continues to mention her son who is in foster care, and the physical altercation she sustained many months ago here at this shelter. SC received VM form Tanya, ERS provider via Active Aid Solution. Tanya reported that they received a monthly maintenance service order for ERS but did not get one for the installation and requested that SC put it in ASAP. SC created service order in Oracle and completed service order in SAMS. SC reviewed Pa care plan and everything else looks good. SC placed phone call the Penn Asian Senior Services enhanced day care provider. SC call was transferred to Soon Pack Pa’s social worker at the center. When Soon got on the phone SC introduce self and stated reason for calling (was to visit Pa at the day center) Soon stated that, that would be ok because SC must be there before Pa’s leaves at 1:45 PM. SC asked why Pa leaves so early and Soon reported that, that’s
Iraida (PSA) received a call from Ileana who requested the HCSS to leave. As per Ileana, the HCSS had an attitude and they weren’t getting along. She requested the HCSS to leave her at her MD office and she’d be taken home by transportation. Ileana alleged the HCSS was talking badly about her to the MD office staff (HCSS denies this). Ileana was informed that we wouldn’t be able to leave her alone. SC was informed of situation and attempted to speak to Ileana over the phone. Ileana couldn’t speak since she was being seen by the MD. Aide was able to redirect Ileana and calm her down. She allowed the HCSS to accompany her back home. HCSS is refusing to return to the case.
On this date, Matthew Mitchell APS supervisor conducted a meeting on behalf of Mr. and Mrs. Beaird well-being. In the meeting, Mr. Mitchell presented Mr. Joey Beaird and Mrs. Julie Johnson with solutions that would allow Mr. Gail Beaird to return home. However, both Mr. Joey Beaird and Mrs. Johnson declined to take care of their parent independently. Each stated their home was not handicap assessable and they had jobs and could not dedicate that much time to their parents care. The other option discussed was hiring another service besides Visiting Angels since they are refusing to continue services if Mr. Beaird returns home. However, this is the only service offered in Jasper. When the discussion of Mrs. Johnson and Mr. Beaird hiring sitter independently they stated they had tried this before and it never worked out.
CCIB Intake received a call from Stephanie from a law office located at 3250 Wilshire Blvd., Los Angeles (323) 939-0506. Stephanie stated resident La Sheta Hayes DOB: 6/24/69 arrived at the office at approximately 3:30PM on 4/12/16. The resident informed Stephanie that she had returned from her Beverly Hills law office in attempt to retrieve her file. The resident disclosed that she was in a car accident several years ago. The resident arrived on bus #20 using a walker. She appeared confused and disorientated. Stephanie believed the resident should have an escort due requiring a walking and her confusion. Stephanie disclosed the law office could not assist the resident with her issues. The resident waited in the office over an hour before leaving.
CCIB received SOC 341 reporting that during a meeting with the resident, James Brown (DOB: 09/18/84), social worker from Solano Adult Day Healthcare Center, the resident stated that staff Marco has hit him on multiple occasions. The resident kept repeating that he did not want to return to the facility. RP consulted with resident's previous social worker who affirmed that the resident has a history of making allegations that are not true due. Resident also has a history of Schizophrenia/Delusions. Facility staff stated that the resident has a history of making allegations whenever he is assigned a new social
Reporting party (RP) stated he arrived at the facility to meet with resident George Dixon (7/11/52) on 3/29/17. Resident was recently placed at the facility after being released from the hospital. Staff did not allow RP to enter the facility so that he could conduct his metal health assessment for the resident. RP presented his business card from his agency but was still denied entry. The resident has been enrolled in RP's program since 02/01/16 and has been visited previously by Social Worker Kathy Kleinman and by staff Denise Ramirez on 3/10/17. Staff told RP that he has to contact the administrator to make an appointment to visit with resident. RP's agency faxed over the consent for treatment to the facility and the DMH contract. Patient's
Maria yesterday, I met with Ms. Betty Blake, she has two letters from the Dialysis Center that she goes to. One of the letters is from the doctor and the other letter is from the Social Worker. Both letters are addressed to the Department of Homeless Services, requesting for the client to be transferred to a medical shelter. Client refuses for this worker to make a copy of both letters. The Social Worker letter stated that in the past the client was physically assaulted at this shelter and this shelter is not a good setting for the client.

The reporting party (RP) stated on 4/1/16 resident Clifford Soares exited the building and was found by the local Police Department and returned at approximately 2:00AM. According to the RP the resident lived in the memory care section of the facility. When the resident exited the facility the alarm didn't sound or the caregivers did not hear the alarm sound. The RP stated the facility failed to secure and monitor residents in the Memory Care Unit.
