Introduction:
High blood pressure, also called hypertension, the blood pressure in the arteries is raised.
Hypertension of two main factors, which may be present individually or together:
The heart pumps blood with excessive force.
Smallest blood vessels of the body (arterioles) narrow, so that blood flow exerts more pressure against the walls of vessels.
(Simon.H, 2006)
Definition of hypertension:
Rise of blood pressure is common in long-term strength of the blood on the walls of the arteries is high enough case that it may finally lead to health problems, such as heart disease.
Blood pressure is determined by the amount of blood the heart pumps and the amount of resistance to blood flow in the arteries. When systolic is above140mm of hg

Nursing interventions:
• Maintain bed rest, quiet neighborhood, a little light.
• Limit of patients in the activity.
• Minimize disruption and environmental stimuli.
• Give a fun action according to indications such as ice packs, the position of comfort, relaxation techniques, counseling imagination, avoid constipation.
Expected Outcomes:
Patients revealed the absence of headache and looked comfortable
2. Decreased cardiac output
Nursing diagnosis:
Drop cardiac output related to increased afterload, vasoconstriction, myocardial ischemia, ventricular hypertrophy.
Nursing interventions:
• Monitor and measure blood pressure in both hands, using proper techniques
• Auscultation of breath sounds and heart tone. Observe skin color, moisture
• Maintain rest in bed or chair.
• Assist in performing self-care activities as needed.
• Provide a quiet and proper environment
Expected outcomes:
• Maintaining blood pressure within an acceptable range.
• Maintaining stable cardiac rhythm and frequency
3. Deficient Knowledge
Nursing diagnosis:
Deficient Knowledge related to lack of information about the disease process and self-care.

Hypertension, as we all know the term High Blood Pressure, is defined as a systolic pressure greater than 140 mm Hg and the diastolic pressure greater than 90 mm Hg. Hypertension is a Chronic medical condition that occurs when blood flow in the arteries are high causing artery walls to narrow and thin, blood pressure is the result of two forces the amount of blood measured being pumped out of the heart and into the arteries and the amount of time heart rest between each heartbeat . Systolic pressure occurs when the left ventricle is mostly contracted, the diastolic preforms when the left
Conditions such as hypertension and coronary artery disease cause an increase in blood pressure. Blood pressure increases when arteries become narrow as a result of hardening or due to build up of fat deposits; both of which are major
Hypertension is when the systolic pressure exceeds 140 mmHg or even if diastolic exceeds 90 mmHg. Hypertension occurs when the blood vessels walls become thicker and less elastic, which causes less blood and oxygen to flow to all body systems. When the blood vessels become more constricted due to the loss of elasticity peripheral vascular resistance begins to increase causing more pressure to occur within the vessel walls. With more pressure and blood vessels becoming more constricted, less blood can reach all systems of the body as the heart is working harder to pump blood.
The reality is that high blood pressure is a condition that makes the heart work harder than normal and if left untreated, it scars and damages your arteries and can lead to heart attack, stroke, kidney failure, eye damage, heart failure and fatty build ups in the arteries, called atherosclerosis.
Blood pressure is the force exerted by the blood on the inner walls of the veins/vessels. In many cases, when blood pressure is too high a multitude of chronic diseases can occur such as heart disease, atherosclerosis, and negatively impact every blood vessel that travels around your body. According to your blood pressure results (High Blood Pressure stage 1 Hypertension), you are in the unhealthy range putting you at risk for these diseases and others like them. This can be a result of sedentary lifestyle, poor diet, lack of exercise, smoking, drinking and etc… Blood pressure is typically inconsistent, for example is can vary by just walking around or by being nervous but your results indicate that it may be high regardless. However, it is
Having hypertension and leaving it untreated is very serious, and can lead to many serious health issues from heart attack, kidney failure, stroke even death. Hypertension increases the pressure of blood that goes through your arteries, which over time will weaken them and can cause a bulge that can lead to an aneurism. It can also cause a stroke due to the arteries not allowing enough oxygenated blood through. Hypertension causes damage to your arteries and blood vessels that it can also cause kidney damage and disease a where from kidney failure to kidney scaring and kidney artery aneurism. There are many things that can happen if you leave your blood pressure unattended to. Mainly it must do with your arteries and blood vessels due to the excess pressure that your blood pumps into the vessels and leaves less oxygenated blood which leads to issues with your brain.
Hypertension is commonly known as high blood pressure. What does it mean to have high blood pressure? Blood pressure can be defined as the amount of blood the heart pumps and the amount of resistance to blood flow in the arteries. It is when the systolic reading is greater than 140 and the diastolic is greater than 90. Systolic is blood pressure when the heart contracts while diastolic is the time when the heart is relaxed. Primary care physicians are accustomed to seeing cases of hypertension. However this common diagnosis can lead to more serious illnesses such as chronic kidney disease, stroke, heart attack, aneurysm, peripheral artery disease, and even death (Tipton & Sullivan, 2014).
Hypertension is a very common condition that affects many people in the world . Hypertension is also known as high blood pressure and it is when your blood pressure is above normal. Around 75 million people in the world suffer from hypertension. Hypertension can affect anyone in the world. That shows how common this condition really is. Hypertension happens when the body's smaller blood vessels narrow, which causes the blood to have a lot of extra pressure against the vessels wall, which makes the heart work harder. High blood pressure is usually identified when the blood pressure succeeds 140 over 90 while the normal average blood pressure is 120 over 80. This is bad for the heart because too much pressure can lead to other conditions like
If the numbers measuring your blood pressure stay above normal most of the time, you are at risk for health problems. Hypertension, commonly called high blood pressure, is a long-term (chronic) condition in which blood pressure is elevated. This condition often has no signs or symptoms. The cause of the condition is usually not known.
Foods for high blood pressure reduction include unsalted foods, low fat foods and foods high in potassium and foods that are high in fiber. Sodium is an enemy to hypertension. By itself, a high sodium diet will usually elevate blood pressure readings. Foods that are high in potassium tend to neutralize or even lower levels of sodium. So, can we put this all together into a diet that will help lower blood pressure? Yes, we can, and in this article, we will do so.
On 9/9/2015, at approximately, 11am, CM met with Esther Toro in the cafeteria, to follow up on Ms. Toro High Blood pressure medication. Ms. Toro was sitting in the Cafeteria and she seems disoriented. She wasn’t oriented to person, place, time and situation, because client stated she didn’t went to the emergency room yesterday. She continues to mentioned she went last week. CM asked Ms. Toro if she was able to pick up her blood pressure medication yesterday, September 8, 20015. Ms. Toro replies “yes” she picked up her blood pressure medication. CM asked to see the blood pressure medication bottle. Ms. Toro shows CM the blood pressure medication bottle with only 9 pills. CM mentioned to the client the bottle stated that she received 30 pills
Hypertension (High Blood Pressure) is a sickness that a great many individuals of different varieties are influenced by. In spite of the fact that stress and emotional pressure can incidentally expand pulse; however it would not be considered as hypertension. Hypertension normally is there with no manifestations. It may happen and stay inconspicuous for a long time. Individuals, until told by a therapeutic doctor by large disregard hypertension. Individuals are not mindful of how hypertension influences the body, how it is measured, what the side effects are of hypertension, and what individuals/ethnic gatherings are influenced by hypertension more than an alternate. Late research has uncovered that under diagnosed hypertension influences millions and a huge number of individuals extending from youths to the elderly.
High blood pressure (HBP), otherwise known as hypertension is one of the many diseases of the cardiovascular system. Depending on the amount of the blood that is being
While most people know that high blood pressure is dangerous, many are unaware of what causes this problem. Normally, the heart pumps blood throughout the body. Blood pressure is the measure of how much blood is pushed against the blood vessel walls. If someone has hypertension, it means that the heart is working harder to push blood through the body. Left untreated, this can lead to strokes, heart failure, hardening of the arteries and kidney disease.
Blood pressure can be taken in a doctor’s office, a pharmacy or even at home. A cuff is placed around a limb, most commonly the upper arm, and inflated. While the cuff is being deflated, a stethoscope is placed over an artery and used to listen to the heartbeat while watching a gauge. Blood pressure is measured using two numbers. The first number, systolic, measures the pressure in your blood vessels when your heart beats. This is the first beat the listener will hear. The second number, diastolic, measures the pressure in your blood vessels when your heart rests between beats (CDC, 2016). This is the last beat heard while deflating. A blood pressure of 120/80 mmHg is most desired. Anything higher or lower should be discussed with a healthcare
