On this date worker received a call from Ms. Val Wilson, RN at Alacare HH. Ms. Wilson has been Mrs. Brown’s nurse and been in the home multiple times. During Ms. Tara Brown time as caregiver the home was clean, food cooked, and Ms. Brown received her medication. According to Ms. Wilson Tara moved in during the summer after Larry Brown, son of Ms. Brown locked Ms. Brown out of her home. Tara told Ms. Wilson that her father, Larry Brown was MR and suffered from Schizophrenia. She also reported seeing bruises on Mrs. Brown’s arm where Mr. Brown would grab her. On this date worker made an unannounced visit to the residence of Mr. Lawson Lovett, for the purpose of linking Mr. Bobby Lovett with VA nursing home application and information. When worker arrived, Mr. Bobby Lovett was cutting grass. Before worker could exit vehicle Mr. Bobby Lovett came to worker's car. Worker exited the vehicle and explained the reason for her visit. When worker gave Mr. Bobby Lovett the nursing home application he gave it back to worker and asked worker if she could help him fill out application because he did not have much education. Worker explained to Mr. Bobby Lovett she would assist with the application. However, he would need to gather documentation and sign application for Mr. Lawson Lovett since he was POA. Mr. Lovett gave worker his number and told her to call, they would set up a time to fill out
On this date, APS supervisor talked with worker at Amedisys HH in Mobile Co. Amedisys called to report Mr. Parker was left alone in his caregivers apartment since Saturday and she does not plan to return for two weeks. According to Amedisys Mr. Parker is receiving speech therapy, physical therapy, and nursing services. The apartment is two levels and his bedroom is upstairs. Yesterday they believe he took the wrong medication and overmedicated himself and the home was smoky where he tried to cook food. He is also isolated in a dangerous area with no phone service. According to HH, they have called Ms. Nash, the caregiver, three times and she has not returned any calls. APS supervisor referred Amedisys to Mobile Co.
On this date, Matthew Mitchell APS supervisor conducted a meeting on behalf of Mr. and Mrs. Beaird well-being. In the meeting, Mr. Mitchell presented Mr. Joey Beaird and Mrs. Julie Johnson with solutions that would allow Mr. Gail Beaird to return home. However, both Mr. Joey Beaird and Mrs. Johnson declined to take care of their parent independently. Each stated their home was not handicap assessable and they had jobs and could not dedicate that much time to their parents care. The other option discussed was hiring another service besides Visiting Angels since they are refusing to continue services if Mr. Beaird returns home. However, this is the only service offered in Jasper. When the discussion of Mrs. Johnson and Mr. Beaird hiring sitter independently they stated they had tried this before and it never worked out.
The reporting party (RP) stated she is the conservator for her nephew Michael McKinnon is not allowed to make or receive phone calls. According to the RP the facility staff members are to assist Michael to make a telephone call to his family every Sunday. The RP stated the facility staff refuse to assist Michael to use the phone. The RP stated the phone is located in the kitchen which is off limits to the residents. The RP stated she received a text from the facility stating that Michael is receiving a 30 day notice. The RP stated the facility is retaliating against Michael due to persistent involvement in his care. On 09/22/16 I AP Associate Vanessa McClary was informed by CSM Vondra Bradley of a $100 cash shortage, which occurred on 09/20/16 in store 648 between the times of 4:31 pm and 8:39 pm on register 10. Mrs. Bradley also provided me with a journal. After receiving the journal I made my way to the AP office where I began to review video for this shortage. At approx. 6:56 pm I observed cashier Shanice Ellis ringing a customer out on register 10. The customer's total came to $97.74. When it was time for a form of payment the customer used a Debit card, the customer also asked for $200 cash back. The reporting party (RP) stated the foster child arrived to the hospital via ambulance on 9/21/15 at 7PM due to seizure disorder. The foster parents were not present during the transport to the hospital. The RP was concern regarding no bedside presence from foster parents. The foster child was a drug baby resulting in seizure disorder and withdraws. The RP stated the foster child was placed in the home on 9/10/15. The foster mother delayed her arrival to the hospital due to being tired. The foster mother did arrive at 8:40PM and informed the RN at 9:34PM that she wanted to go home. On the event date above, this writer conducted a visit at the Ms. Clearly resident. Present in the home were Trevaughn, Ms. Clearly, and Latrell. During the course of the visit, Ms. Clearly apologized for the missed home visit and provided her interpretation of her involvement with ACS. Ms. Clearly further provided a background about the children father’s and her struggle of obtaining child support for Trevaughn. Latrell father as Ms. Clearly provides some assistance for his son. Ms. Clearly then reports that she does know any information on Trevaughn father whereabouts and the only information she has on him is based on his Facebook page. Ms. Clearly does not communicate with Treavaughn father through Facebook due to ego and she feels that
This paper examines the case study about Sarah Burke. A case study analysis form was completed and symptoms were identified. Identification of certain diagnostic criteria were interpreted and clarified for the exhibiting individual. Illumination of criteria for proposed diagnosis lead to the discussion which resolves the risk factors and clinical features associated with the diagnosis. Recognition of certain symptoms and criteria brought to light other possible comorbidities. Cumulative risk theory, as well as, the diathesis stress model were investigated for possible involvement in the Burke disorder development. Multidimensional factors related to the onset and maintenance of her symptoms are deliberated; as well as, how her culture played a role. In the end, a conclusion is given about suggested pre- and post- treatment, overall benefits of those treatments, and prognosis outlooks.
RT came to the MOP office early to meet with writer and was not aware of his appointment with Dr. Vukasin. However, RT stayed at the TSI office until he met with Dr. Vukasin. RT wasn't too happy about sticking around, but writer encouraged RT to stick around and informed him that it is important that he attends his appointments. Writer inquired about RT's physical and RT informed writer that he still has not got in contact with his PCP. RT provided writer with the telephone number for Hillside Focus Care located at 170-06 Hillside avenue, Jamaica NY 11432. On 12/28/16 Mr. Rizzotti has express that his cell phone has not been working for several weeks. He came to the staff office and Wellness Coach Teresa Cain provided him the number and resources to recover a new cell phone. Mr. Rizzotti called his cell phone company to express that he phone was no longer working. In completion of that phone call Mr. Rizzotti stated that they will be providing him with a new phone in the upcoming weeks. Wellness Coach then asked Mr. Rizzotti about his medical appointments and he stated that he will rescheduled them when he get a chance. The case of Kathryn Mc Neil deals with the issue of separation in the workplace regarding the nonperformance of a single mother whose work was suffering because of the tremendous additional burden of raising a child. The case was written in 1994 but in the intervening years significant decisions have been made by the law (in the US) protecting the rights of the single mother. The debate is no longer exclusively about professionalism versus corporate responsibility but the new phenomenon of a third gender in the workplace.
